  • 1. Digital MDF using Google Docs
  • 2. Creating the Form
  • 3. by requiring sign in to view form, students are “digitally signing” the form. Username is also collected automatically
  • 4. link used to put on a web page custom message will show up after student completes the form

    • Form as it’s own “page” accessed via link on blog/email/webpage
    • Form opens in new tab or window

  • 8. information entered on form submits as a spreadsheet in google docs because username is collected automatically, form could be produced w/o a place for “name” making completion quicker
  • 9. form can be downloaded as excel doc and merged with current excel documents

    • Issue #1: student log in on a teacher domain created form will not go through, resulting in useless form
    • Solution: form admin would need a student domain name (mvmdf) to cut down on “tech issues” for would be managed and created from new domain
  • 11.
    • Issue #2: where will student complete form if there is only one computer in the room?
    • Solution: students can fill out form from teacher computer. MDF can be opened and sitting at the ready in a new window or tab
