  1. 1. case presentation<br />Dr PAVAMAN GOTKHINDI . BDS SHAMBHAVI DENTAL CLINIC. . Jamkhandi <br />A 27 yr old female patient visited my clinic with history of tooth ache since 1 month,in the lower left back region of mouth. O/E a periodontal pocket was present between tooth no 36 and 37.<br />Lower second molar(37) was mobile(gr2) and was tender on purcussion. Furthermore 37 was showing signs of fusion. Because of the improper size and positioning of tooth there was a diastema between first and second molars,which caused the localised periodontitis leading to periapical abscess.<br />Two treatment possibilities were explained, which are-<br /> - root canal treatment with crown to eliminate diastema.<br /> - extraction followed by fpd.<br />Patient opted second treatment. The tooth no 37 was extracted.<br />Examination of extracted tooth revealed , it was a fused second molar with a supernumerary tooth. Fusion is a rare condition in permanent dentition that too in posteriors.<br /> •occlusal view of fused second molar(37). <br />•See the incomplete fusion of crowns,with supernumerary tooth resembling premolar.<br />• Buccal view<br />• from the above picture it is evident that the root of supernumerary tooth is fused with the mesial root of second molar.<br />•Rediographic evaluation-<br />The radigraphic examination revealed that the fusion was incomplete with seperate pulp chambers of second molar and supernumerary tooth.<br />• To conclude, it was a rare case of Fusion affecting the lower second molar unilaterally.<br />THIS IS A MS DOC FILE CREATED ENTIRELY ON MY ANDROID PHONE.<br />. pavaman<br />
