  4. 4. Aim <ul><li>Richland Community Health Care will redesign it’s current health care system for patients with Diabetes to reduce mortality associated with this chronic disease. We will do this with the implementation of the Care Model and will use the national measures provided to determine effectiveness. </li></ul>

Population of Focus <ul><li>150 patients in care of Dr. Gold. </li></ul><ul><li>These will be 150 patients diagnosed with diabetes. </li></ul><ul><li>Pt’s selected to be a part of this program to be given a chance to fully improve their lifestyle despite discrepancies brought on because of their diagnosis. </li></ul>

Key Diabetes Measures < 7.0 Average HbA1c >70% If have integrated dental clinic: Dental exam in past year >40% >70% Patients with BP <130/80 Patients with LDL<100 >60% >75% >80% <ul><li>Cardiac Risk Reduction: </li></ul><ul><li>Option 1 Statins </li></ul><ul><li>Option 2 ACE inhibitors or ARB medication </li></ul><ul><li>Option 3 Aspirin or other antithrombotic agent use </li></ul>>70% Documentation of self-management goal setting >90% Patients with 2 HbA1c’s in last year (at least 3 months apart) Current Status as of 02/05 Goal Measure
