  7. 8. “ LINKS” Project. <ul><li>Questions? </li></ul><ul><li>How many people on average require to be signposted to a “community resource”. </li></ul><ul><li>What type of resource is most in need? </li></ul><ul><li>How acceptable is the recommendation to be signposted? </li></ul><ul><li>How easy is it for GP practices to collect and use the information? </li></ul>
  8. 9. “ LINKS” Project. Approx 20% of consultations with GP or Nurse required signposting to Community resources. Approx 70 - 80% of recommendations were then made by GP or Nurse. 50-60% of people accepted the recommendation to the resource.

  11. 12. “ LINKS” Project. <ul><li>Key Learning Points? </li></ul><ul><li>All practices now aware of large range of support resources in their local areas and are enthusiastic to use them. </li></ul><ul><li>Most practices have taken time to visit the resources or invite them into the practice. </li></ul><ul><li>Improved relationships and knowledge will increase the use of these resources. </li></ul><ul><li>Time is required to develop and update the practice directories of resources, how can this be sustained. </li></ul>
