System Overview <ul><li>Our Continuum of Care is operated in four counties in Northwest Ohio– Wood, Sandusky, Ottawa and Seneca </li></ul><ul><li>Each county has monthly meetings with designated officers </li></ul><ul><li>Each county identifies their community’s specific needs </li></ul><ul><li>Each county has separate goals and objectives that all relate to the goal of ending homelessness </li></ul>

How the Continuum Works: Updates, funding opportunities and requests for additional data are provided from the state level Data and accomplishments are shared with advocacy groups and funding sources Information from each CoC is compiled and submitted Data is shared ideas are discussed at Coalition Meetings In Each County
