Project Summary / DMAIC Format Description
The following worksheets outline the key information typically included in a DMAIC Six Sigma report.
DMAIC Definition: Define / Measure / Analyze / Implement / Control
The intent of this documentation is to fully document the testing methods, data analysis, and recommendations for the project.
The following is a summary of the worksheet pages included in this report.

Process mapping is the method for documenting the process steps, required inputs, and desired outcomes. The process map is a critical step in accurately defining all the
variables within a process. This information will be utilized when developing the FMEA. The FMEA takes the inverse of the desired outcome for each process step and labels this as
the 'potential failure mode'. The FMEA then takes each required input from the process map and relables this as a potential cause for the failure mode.
The process map and FMEA documents are linked together in this fashion. The FMEA will only be as good as the resolution of the process map. The process map must include all
the critical infomation regarding the process.
The process map is an excellent tool to develop a solid understanding of a new process. The work in developing a thorough process map is upfront in a project. This may be time
consuming in the early stages of the project but will generate benefits (and reduce risk) later in the project.
Severity (SEV): Severity regards the impact to the project if the failure mode occures
Score: 1 (not severe) to 5 (very severe)
Occurrence (OCC): Occurrence regards how often this failure mode may occur
Score: 1 (not frequent) to 5 (very frequent)
FMEA for Posting Videos to YouTube: The standard FMEA (left side) is sorted by process step
Detection (DET): Detection regards how likely the failure mode can be caught to prevent escape
order (step 1...then step 2...etc.). The RPN sorted FMEA
Score: 1 (controls will detect failure) to 5 (no detection capability)
RPN: RPN represents total RISK for each line item (RPN = SEV x OCC x DET) (on right side) is sorted from highest RPN value (highest
Score: Higher RPN values represent higher risk
risk) to lowest RPN value (lowest risk). This formt quickly
highlights the areas of risk where additional attention
and/or controls may be needed.

Control Plan:
Objective: Control plan documents how to \"lock in\" gains realized from the project recommendations
Strategy:
1. Standard approach is to target documentation report and other initiatives such as training
2. The control plan for this project incluces the structured reports generated - DMAIC report and presentation
3. Publish results to Slideshare for further discussion with readers
4. Use this project as a building block for additional studies using the same tools
5. Document results and request critical feeback from readers
