The penile implant is placed surgerically either through one incision under the scrotum or through an incision under the lower abdomen.
Usually the device is filled with saline and the patient has a pump which is hidden under the scrotum. By pressing on the pump the saline travels through a reservoir and travels into the device. (University 2011)

Like any other medical procedure there are risks and unsuccessful surgeries; however, it seems this procedure has been refined and the majority of the “kinks” have been worked out.
