Upon the conclusion of the visit, the care provider is provided an opportunity to enter a journal entry which describes the provided treatment. The patient may obtain a copy of the treatment history and fax it to the DSA/M3 data center. The received copies of medical treatment summaries will be appended to the patient’s on-line medical record.
