    2. How to add an E-mail Form Begin with the Content Editor open…. Step One Place your cursor where you want the form to begin. Step Two Click the E-mail Form Builder button
    3. How to add an E-mail Form The E-mail Form Builder tool will display. Delivery The top part creates the delivery information for the form. Add Form Fields The lower part creates the fields for the form. Insert Form Click Insert Form when you are done. Clear All Fields Clears the fields so you can start over.
    4. Adding Delivery Information To Enter the e-mail address you want the form sent to. Subject Enter the subject for the e-mail. This is the title of the e-mail that will be sent by the form. Submit URL Enter the URL (address) of the page you want the user to be redirected to when they submit the form. Make the selection from the dropdown menu Button Text Enter the word you would like to display in the button. The default is “Submit”.
    5. Adding Form Fields Name Enter the name of the field you would like to create – this is what will display next to the field.* Type Select the type of field from the dropdown menu. This is covered in the next slide. initial Value Enter the text you would like to display in the field. Most people leave this blank. Add Click Add when you a done entering your selections for this field. You can add as many fields as you need by repeating this process for each field. *Spaces and unacceptable characters will be filtered out when entering the Name. You can format the form later if this is not the result you want.
    6. Adding Form Fields There are three types of fields to choose from: -Text Box -Text Area -Checkbox Examples of each field type.
    7. Example The settings of this Email Form Builder…. created this form…. that sent this e-mail.
    8. Helpful Tips
      • Create your Submit URL page prior to creating a form. (example: submit success)
      • Be sure to click the Insert Form button when you are done adding your fields.
      • Once the form is inserted onto the page you can edit the titles, add missing spaces and do other formatting to make it look the way you want it to.
      • The Email Form Builder works best using Internet Explorer
