Definition of Illness and Injury (cont.) <ul><li>Immediate Family Members: </li></ul><ul><li>Mother, father, husband, wife </li></ul><ul><li>Domestic partner (registered with HR) </li></ul><ul><li>Brother, sister (blood, step or half) </li></ul><ul><li>Son, daughter (blood, step or adopted) </li></ul><ul><li>Father or mother-in-law </li></ul><ul><li>Daughter or son-in-law </li></ul><ul><li>Grandparents, grandchildren </li></ul>

Notification Procedures <ul><li>Notify immediate or appropriate supervisor </li></ul><ul><ul><li>In person or by telephone (no voicemail) </li></ul></ul><ul><ul><li>Within 30 minutes of start time </li></ul></ul><ul><li>If notification is not within required time, absence should be without pay, unless employee can later substantiate and document that it was impossible to make or cause such notification </li></ul>
