INTRO (CON’T) Nephrons filter the blood to remove waste and selectively remove water, electrolytes  They filter out the ions (K, Na) and water that are required by the body & return them to bloodstream  Takes place in the tubules   Proximal, loop of henle, distal Those substances not needed are excreted as urine  Bladder is storage area 
  4. 4. DIURETICS Drugs to reduce edema  Five Classes       Carbonic anhydrase inhibitors Loop diuretics Osmotic diuretics Potassium sparing diuretics Thiazides

ACTIONS Most have the action of increasing the excretion of sodium and potassium in the renal tubules  Potassium Sparing diuretics “spare” the potassium  The save potassium, while excreting sodium  Act by depression excretion of potassium  Antagonizes (blocks action of aldosterone  Aldosterone is salt retaining hormone 
  6. 6. USES Edema w Heart failure, steroid therapy, cirrhosis of liver  Hypertension  Renal disease  Cerebral edema  IOP with glaucoma 
  7. 7. SIDE EFFECTS Dizziness  orthostatic hypotension  GI upset  Hypokalemia s/s  Muscle cramps  Weakness  arrhythmias   Hyperkalemia (K+ sparing) weakness  arrhythmias 
  8. 8. CONTRAINDICATED In clients with anuria  K+sparing CI in hyperkalemia and kids   Used cautiously with renal dysfunction
  9. 9. NURSING PROCESS  Baseline  VS  Weight  Edema  I&0 Labs   BUN  Creatinine  Clearance Nsg Diagnoses  Impaired uinary elimination  r/o deficient fluid volume  R/O injury
  10. 10. NURSING PROCESS Administer early in day  Potassium supplements  Avoid high potassium foods and salt substitutes if taking K+ sparing diuretic 
