What you can do now to prepare <ul><li>Use Kinnser ’s dashboards to get asses the scope of outstanding documentation </li></ul><ul><li>Ensure that your agency is using the Kinnser flags accurately </li></ul><ul><li>DC patient charts need review </li></ul><ul><li>Ensure personnel file licensures are up to date </li></ul><ul><li>Use key reports in Reports/Admin section to check for areas of vulnerability </li></ul><ul><li>Consider a mock survey, using auditors who know Kinnser </li></ul><ul><li>Use the remote capabilities to ensure changes are made </li></ul>

How to use Kinnser during survey <ul><li>Pulling an unduplicated census </li></ul><ul><li>Click on Go To </li></ul><ul><ul><li>Patient Manager or Reports/Admin </li></ul></ul><ul><ul><li>Click Create Patient Roster </li></ul></ul><ul><ul><li>Scroll down the patient roster form </li></ul></ul><ul><ul><li>Select option no.3 and enter the date range that you need </li></ul></ul><ul><ul><li>Click Create Roster </li></ul></ul><ul><ul><li>Print roster for surveyor </li></ul></ul>

How to use Kinnser during survey <ul><li>Printing a patient chart </li></ul><ul><li>Click on Go To </li></ul><ul><ul><li>Patient Manager </li></ul></ul><ul><ul><li>Click on the first letter of the </li></ul></ul><ul><ul><li>last name to open the chart </li></ul></ul><ul><ul><li>Go to the episode requested </li></ul></ul><ul><ul><li>and click on Tools then Print Chart </li></ul></ul><ul><ul><li>Repeat process until all charts requested have been printed </li></ul></ul>

Limit your exposure <ul><li>Delegate someone in the agency to remove the yellow stickies if you feel that’s necessary (some agencies leave them on as training opportunities for nursing staff on documentation) </li></ul><ul><li>Have your internal auditor or consultant look for “high profile” patients </li></ul><ul><li>Call in outstanding notes from field staff </li></ul><ul><li>Send a K-mail informing staff that agency is under survey </li></ul>

What to do if your surveyor wants a log in <ul><li>You might offer to print the charts, but the surveyor can request a log in </li></ul><ul><li>Create a user name and password for the surveyor </li></ul><ul><li>Recommended roles to give surveyor: </li></ul><ul><li>Clinical Review and Clinician </li></ul><ul><li>Set up access hours for the </li></ul><ul><li>surveyor under Edit  Profile </li></ul><ul><li>Inactivate User promptly after the exit interview </li></ul>

G156 Acceptance of patients, Plan of care, and Medical Supervision </li></ul><ul><li>Not Admit Patient </li></ul><ul><li>Surveyors will often ask to review your not admit files, Make sure your narrative documentation includes that the referral source was notified of non admit/why </li></ul><ul><li>Plan of care must be developed, followed, reviewed, and updated </li></ul><ul><li>2.
