The school’s proposed increase in tuition and other school fees are shown on the table below:
For this application to be approved, the school hereby submits the following documents:
1. Endorsement of the Schools Division Superintendent
2. Standard Letter-request asking for confirmation of tuition and/or other fees (QAAD Form No. 5)
3. Certification under oath signed by the school head that the following requirements of R.A. 6728 have been complied with, namely, that:
• appropriate consultations have been conducted by the school administration with duly organized parents and teachers associations and faculty
associations, or their equivalents in the absence of such organizations, not later than March 30,
• (70%) of the said increase must go to the payment of salaries, wages, allowances, and other benefits of teaching and non-teaching personnel
(not needed if the increase proposed is only for other school fees);
• at least 20% of the said increase must go to the improvement and modernization of buildings, equipment, libraries, laboratories, gymnasia and
similar facilities and to the payment of other costs of operations, and
• full or half tuition fee waivers, or their equivalents
4. Breakdown of Other Fees for the forthcoming school year
5. Copy of previous confirmation of tuition and other fees
6. Copy of Government Temporary Permit or Government Recognition
