The Osteopathic Lesion The official definition of the faculty of the European School of Osteopathy - Klug et al (October 1990)

An Osteopathic Lesion is a mechanical and physiological reaction in somatic structures to:
Trauma
Sustained or recurrent mechanical stress
Disturbed visceral function/disease
Psycho-emotional stress

All of which may lead to any one of the following:
Pain
Abnormal sensation
Reduced physical capacity
Organic disease
Any functional disorder

Littlejohn (1917) ‘ Any alteration in the anatomical or physiological relationships yet within the normal range of movement of articular structures, resulting in a local or remote functional disturbance’.

American Osteopathic Association (1933) ‘ Any pathological variation from the normal of the position, soft tissue support and mobility of one or more skeletal structures which originates or registers symptomatology or pathology’.

Alan Stoddard (1969) ‘ This is a condition of impaired mobility in an intervertebral joint, in which there may or may not be altered positional relations of adjacent vertebrae. But when altered position is present it is always within the normal range of movement of that joint’.

Osteopathic Lesion/Somatic Dysfunction Due to the confusion regarding the term ‘lesion’ we now refer to an osteopathic lesion as a somatic dysfunction.

The Somatic Dysfunction as defined by the Educational Council on Osteopathic Principles: American Association of Colleges of Osteopathic Medicine (2002) ‘an impaired or altered function of related components of the somatic framework and related vascular, lymphatic and neural elements’

… for the purposes of THIS lecture… …we will think of it as, simply, ‘dysfunction’.

Lesion Terminology…
‘ Active Movement’
Achieved by oneself
Ceases at the ‘physiological barrier’

Lesion Terminology…
‘ Passive Movement’
Achieved by a third party (osteo? trauma?)
Ceases at the ‘anatomical barrier’

Dysfunction occurs when… …restriction occurs within the normal, physiological range of movement. This is referred to as the ‘restriction barrier’

Fryette’s first law of motion …states that side-bending and rotation occur in opposite directions with the vertebra in neutral

Fryette’s second law of motion … states that side-bending and rotation occur in the same direction with the vertebra in either flexion or extension

First degree lesions
Relate to Fryette’s second law
Are therefore SB and Rot to the same side and are either in Flexion or Extension
Are the most common form of dysfunction

Second degree lesions
Relate to Fryette’s first law
Are therefore SB and Rot to opposite sides and are in neutral
Are the demonstration that the body is possibly trying to ‘balance’ the flexed/extended vertebra by placing it in neutral

Therefore, a second degree lesion is the body’s attempt at correcting a first degree lesion.
Thus, a second degree lesion is more likely to be older than a first degree lesion

Dysfunction Chronicity A qualitative perspective of dysfunction. Purely subjective. Rarely verifiable. Vitally important.

Pathology …can lead to a ‘pathological barrier’. This occurs within the ‘physiological barrier’ but affects all planes of movement.

Direct and Indirect modes of treatment …relate to how, exactly, a treatment modality or principle is used to correct dysfunction

X-Somatic and Somato-X Relate to the pathways and possible ‘links’ between physical dysfunction and other factors.

Primary Lesions
May be osteopathic or pathological
May be the most significant lesion in the body
May be the oldest lesion in the body

Secondary Lesions
Are direct consequences of a primary lesion
May be passive (follow/copy the primary)
May compensate to the primary in opposition to assist harmony and function (eg Scoliosis)
May attempt to correct the primary by acting in direct opposition. These are usually in adjacent structures
