[Independent Living Zone] (4 words max) “ Linking Resources to People & People to Services" thereby benefiting the community: Identifying availability of services and resources to enable disadvantaged and excluded individuals and groups to navigate their way independently, or with assistance, access to social and health care support that reflects the overall developments in Personalisation.

      • Features of your project
      • Aids to Daily Loan Scheme – Try before you buy.
      • Demand Responsive Training integrating disabled people and professionals working towards Inclusiveness.
      • Integrating community assessment and linking to wide range of suppliers across the north east.
      • Efficiencies created
      • By creating Care/Community Equipment Path-ways that involve specialist and mainstream services thereby maximising resources, and mapping gaps in services so as to encourage development of new providers.
      • Needs your project meets
      • Helping people implement Self Directed Support.
      • Increase access to mainstream services in line with diversifying range and scope of disability support services.
      • Create a ‘One Stop Shop’ approach and thereby integrate a wide range of services.
    4. Measures of success
      • Number of disabled people maximising use of Individual Budgets
      • Increase in collaborative working between sectors.
      • Reduction of number of ‘Crisis Intervention’ by statutory authorities.
      Evidence of impact helps enlist support. Describe 3 ways in which you will measure success and demonstrate your idea is making a difference.

      • Research infrastructure comprising of statistical support.
      • Strategic Marketing.
      • Support for disabled people to participate in development of the organisation.

      • I am willing to attend the Festival of Ideas on 12 th November to talk about this project and to be signed up to the Innovation Exchange website to support this knowledge sharing after the event.
      • I do understand that this event is the launch of the North East JIP’s Provider Development Fund and I am interested in the possibility of applying to this Fund in the future
      • I do understand that this Fund will only support projects that involve collaboration between at least two providers of social care services and that funded projects will be part of an evaluation process to include both research interviews and knowledge sharing events.
