Los pacientes habitualmente tomando –antes
del STEMI- antiinflamatorios no esteroideos
(AINE), a excepción de la aspirina, tanto no

la presentación del STEMI, por el aumento de
los riesgos de mortalidad, reinfarto,
hipertensión, insuficiencia cardiaca, infarto y

Anticoagulantes
Pacientes reperfundidos con fibrinolíticos deben
recibir terapia anticoagulante -HNF- por un mínimo de

Clopidogrel 75 mg oral por día debe añadirse a la
aspirina en pacientes con STEMI independientemente de
que sean objeto de reperfusión con terapia fibrinolítica o

Tratamiento con clopidogrel debe continuar por lo menos
14 días.
Todo paciente post ACP que recibe un DES, clopidrogel 75

DOSIS DE CARGA
En pacientes < 75 años quienes reciben terapia
fibrinolítica o no reciben terapia de reperfusión, es
razonable administrar clopidrogel por vía oral en dosis
de carga de 300 mg. (No se dispone de datos para
orientar la toma de decisiones con respecto a una dosis
de carga oral en pacientes ≥ 75 años de edad.)
DURACION
Terapia de mantenimiento de largo tiempo (e.j., 1 año)
con clopidogrel (75 mg oral por día) pude ser beneficioso

DESARROLLAR EL TEMA PROPUESTO POR LA ORGANIZACIÓN DEL CONGRESO ES UN ENORME DESAFÍO CUYAS ARISTAS MAS VISIBLES RADICAN EN A) COLECTAR –Y SOBRETODO- SELECCIONAR LA INFORMACIÓN, QUE PUEDA SER DE UTILIDAD EN UNA DISCUSIÓN CON EXPERTOS COMO LA MAYORIA DE ESTA AUDIENCIA, Y B) Enfocar esa información al ejercicio clínico y a nuevos problemas de investigación.
LOS NUEVOS TIEMPOS Y LA AVALANCHA DE INFORMACIÓN MARCAN UN ABORDAJE QUE INCLUYE UN ENFOQUE TRIPLE. El enfoque de uno solo de ellos, sería un abordaje incompleto y fragmentado.
En primer lugar desearía establecer con ustedes el marco PROBABILÍSTICO contenido en esta CLASIFICACIÖN de RECOMENDACIONES y NIVEL de Evidencia Aplicada., cuyo uso debe generalizarse:
La circulación coronaria es única porque genera su propia Presión de Perfusión (PP)lDiastólica y Resistencia Sistólica. Las coronarias perfunden de epicardio a endocardio con la presión arterial diastólica -80 mmHg-. Si la PFDVI es de 10 mmHg, la PP sería 70 mmHg. (Oferta). Durante la contracción sistólica la Presión de Ao y Vi se igualan -120 mmHg- y anula la PP. Sin embargo en la Contracción Isovolumétrica ocurre el 90% del MV02(Demanda). Este Balance O/D se relaciona a la función contráctil del VI. Una Obstrucción incompleta con disminución del Flujo coronario produce caída de la PP, ISQUEMIA SUBENDOCÄRDICA distal y falla sistólica. Se inicia un circulo vicioso porque aumenta la PFDVI, que aumenta la resistencia al flujo y mayor caída de la PP.
LA AUTOREGULACIÓN CORONARIA previene este círculo vicioso. Mantiene el flujo coronario constante aunque la presión arterial coronaria caiga por debajo de la presión aórtica. Si es que los determinantes del MVO2 se mantienen constantes.(FC, PA, contractilidad). El límite es cuando la PA desciende bajo el rango de autoregulación 30 mm Hg en perros concientes, entonces la resistencia coronaria -ya dilatada al máximo- se vuelve presión dependiente. (Flujo coronario: nl 0.7 a 1 ml/min/gm; Mx 4 a 5 veces). Reserva Coronaria: (MEDS) disminuye con Taquicardia bajo tpo diastólico, aumento precarga, Bajo 02, aumento de contractilidad. La modulación del tono coronario depende del endotelio vascular. Si se extirpa el endotelio la respuesta vasodilatadora se pierde y se produce vasoconstricción. Igual sucede en presencia de Factores de Riesgo Coronario. Ej Acetilcolina vasodilatación vía Oxido Nítrico, NE receptores Beta 2, Adenisina, papaverina, Prostaciclina, Dipiridamole,
It is now recognized that unstable angina (UA), non-Q-wave myocardial infarction (NQMI), and ST-segment elevation myocardial infarction (STE-MI) are all parts of the spectrum of clinical manifestations of acute coronary syndrome (ACS). The older terminology has now been replaced with terminology that divides ACS into non-ST-elevation ACS (NSTE-ACS) and ST-segment-elevation. All the slides in this teaching set deal with NSTE-ACS. La placa de alto riesgo es una continuación de los procesos inmunológicos, inflamtorios y de la cascada de coagulaci´n intrínseca y extrínseca.dando lugar a la disrupción de la placa, fisura, erosión y coagulo. Este proceso da lugar a Dos grandes Sindromes AgudosSin y con elevación de Segemnto ST.
Aquellos con elevación del Segmento ST constituye la tercera parte de las admisiones hospitalarias y son los mas graves. Ese es el grupo que discutiré con ustedes.
Vamos ahora al abordaje de PROTOCOLOS Y GUÏAS. EL EQUIPO DE INVESTIGADORES DEL COLEGIO Y DE LA ASOCIACIÓN AMERICANOS DE CARDIOLOGÍA ME ALIVIA EL TRABAJO DE SELECCIÓN. El reporte de guías clínicas del 2004 sobre le manejo de IMA con Elevación de segmento ST, fue revisado y actualizado el 2007. Yo me permitiré revisar esa actualización con ustedes. Tiene el riesgo de ser incompleto.
LA MORFINA SIGUE SIENDO LA DROGA DE ELECCIÓN EN EL STEMI (CLASE I) Y RECOMENDABLE EL ANGOR INESTABLE SIN ST (CLASE IIa)
SUSPENDER LOS AINES ES UNA INDICACIÓN IC.
Prescribir aines en el stemi esta contraindicado. INDICACIÓN IIIC PORQUE AUMENTA EL RIESGO DE MORTALIDAD REINFARTO HIPERTENSIÓN ICC RUPTURA DE MIOCARDIO.
La aspirina es una INDICACIÖN IC EN EL STEMI. En los casos post STENT. Post STENT la Dosis es mayor y la duración varía de acuerdo al tipo de STENT usado.
Respecto de los Beta Bloqueantes comentaré el primer lugar el Estudio COMMIT.
El Estudio COMMIT demuestra que la administración inicial de metoprolol no afecta eventos en los primeros 28 días aunque aumenta los riesgos de Shock cardiógeno. En cambio, un metanálisis –-que incluye al COMMIT-demuestra mejoría en 6 y 12 meses Este Forest Plot demuestra que la mortalidad mejora en todos los estudios, (Cuociente BB/CONTROL) a 1, Los mas grandes, -el COMMIT- el cuadrado mas grande, y en el total –metanalisis- el rombo abierto se mantienen lejos de la unidad o linea de no diferencia. Los mas pequeños de cuadrado pequeño sobrepasan la linea de no diferencia lo cual demuestra poco poder estadístico. La mortalidad con BB de 4.8% (1265/26189) y en el control fue de 5.5% (1436/26276, lo cual da una diferencia de 87% (4.8/5.5) y una reducción proporcional de 13% estadísticamente significativa. Lo mismo sucede con el re-infarto (2.3/2.8) que es 78% y una reducción del 22%, y en la fibrilación ventricular (3.1/3.6) : 75% y una reducción del 15%
*Risk factors for cardiogenic shock (the greater the number of risk factors present, the higher the risk of developing cardiogenic shock): age > 70 years, systolic blood pressure < 120 mmHg, sinus tachycardia > 110 or heart rate < 60, increased time since onset of symptoms of STEMI. **Porque aparecieron estudios controversiales.
INDICACIÖN IA: Pacientes con IMA con ST, que se presentan dentro de los primeros 90 minutos a un hospital con capacidad para ACP. De lo contrario –ydentro de las primeras 3 horas debe recibir FIBRINOLISIS. Note “ medical contact ” is defined as the “time of EMS arrival on scene” after the patient calls EMS/9-1-1 or the “time of arrival at the emergency department door” (whether PCI-capable or non – PCI-capable hospital) when the patient self-transports.
Planificar una estrategia que incluya FIBRINOLISIS o Bloqueadores de la GP IIb/IIIa plaquetario, seguida de ACP.
METANALISIS DE 17 ESTUDIOS de pacientes –STEMI- con infarto de Miocardio con elevación del ST, randomizados a : ACP facilitada VS ACP Primaria. Aqui observamos los tres FOREST PLOTS que comparan los dos tipos de terapias en cuanto a mortalidad, re infarto y sangrado mayor. En el primero observamos que los estudios que compararon MORTALIDAD, la terapla facilitada con fibrinolíticos, o con IIa/IIIb, o combinada tuvieron mayor mortaldad, solas o sumadas (total u All) que la ACPP primria en un total sumatorio del 38% con un intervalo de confianza que no cruza la unidad o linea de no diferencia, es decir la mortalidad de ACP Facilitada/ACP Primaria es !.38. En cuanto al riesgo de reinfarto, la sumatoria de los estudios evidenció un aumneto del riesgo de 71% con un intervalo de confianza que no cruza la linea de no diferencia y asi mismo el riesgo de sangrado fue 51% mas alto en la terapia facilitada y este tambien alcanzo signifcancia estadistica al no cruzar la linea de no diferencia.
A pesar dl aumento del número de infartos tratados con ACP, la mayoría sigue siendo tratada con FIBRINOLISIS. Cuando esta falla y no consigue una reducción del 50% del ST elevado, se recurre a una ACP de rescate.
En la parte superior del slide (azul), Wijeysundera and colleagues performed a meta-analysis of 8 randomized trials enrolling 1177 patients with failed fibrinolitic therapy for STEMI, to estimate the benefits and risks associated with rescue PCI and repeat fibrinolytic therapy compared vs conservative management. RESCUE PCI (ACP DE RESCATE) was associated with no significant reduction in MORTALITY (relative risk [RR] 0.69 -7.3/10.4-; 95% CI, 0.46-1.05). ACP DE RESCATE was associated with significant risk reductions in HEART FAILURE) (RR 0.73; 95% CI, o.54-1.00)) and REINFARTION (RR 0.58; 95% CI, 0.35-0.97), compared with conservative treatment. Rescue PCI also was associated with an increased risk of stroke (RR 4.98; 95% CI, 1.10-22.5) and minor bleeding (RR 4.58; 95% CI, 2.46-8.55). En la parte inferior del slide (en morado), In 3 trials enrolling 700 patients that reported the composite end point of all-cause mortality, reinfarction, and heart failure, rescue PCI was associated with a significant relative risk reduction of 28% (RR 0.72, 95% CI, 0.59-0.88; P =.001). Further, there was an 11% absolute risk reduction (95% CI, 5%-18%; P <.001) in this composite end point with an incidence of 29.2% in the PCI arm and 41.0% in the conservative arm, leading to a number needed to treat of 9. Repeat fibrinolytic therapy was not associated with significant improvements in all-cause mortality (RR 0.68; 95% CI, 0.41-1.14) or reinfarction (RR 1.79; 95% CI, 0.92-3.48), but was associated with an increased risk for minor bleeding (RR 1.84; 95% CI, 1.06-3.18). Thus, the investigators concluded that rescue PCI is associated with improved clinical outcomes for STEMI patients after failed fibrinolytic therapy, but these benefits must be interpreted in the context of potential risks. On the other hand, repeat fibrinolytic therapy is not associated with significant clinical improvement and may be associated with increased harm. This slide shows the primary endpoint with a highly significant ( P <.001) 17% reduction in the relative risk of death or nonfatal MI in an intention-to-treat analysis. As shown, the curves begin to separate early and then separate more widely beginning after the first 48 hours. There is, however, a reduction of events in the enoxaparin group even during the first 48 hours. The overall results are shown on the left of this slide. The primary endpoint was reduced from 11.2% for control (either placebo or unfractionated heparin) to 9.7% with fondaparinux, a highly significant P value of .008. This was also seen at the earlier time point of 9 days and at the later follow-up of three to six months. There were some differences at the 30 day time period in the results by strata. A significant difference in stratum one (fondaparinux vs placebo) was seen with an absolute 3% reduction in death or MI, whereas in patients who had unfractionated heparin in the control group, there was a nonsignificant difference that went from 8.7% for unfractionated heparin down to 8.3% for fondaparinux. Some of the advantages of unfractionated heparin include immediate anticoagulation with a bolus, multiple sites of action in the clotting cascade, and a long history of successful clinical use. UFH can be readily monitored by either activated partial thromboplastin time (aPTT) or by activated coagulation time (ACT). Some of the disadvantages of UFH include that it is an indirect thrombin inhibitor and thus, does not have the ability to inhibit clot-bound thrombin. It is characterized by areas of nonspecific binding, which can lead to variability in the level of anticoagulation. The inhibition of platelet factor 4 (PF-4) also contributes to variability in anticoagulation. Paradoxically, the long chains on UFH can actually cause platelet aggregation. Other disadvantages include nonlinear pharmacokinetics, which make the anticoagulant response to UFH nonlinear at therapeutic doses, with both the intensity and duration of effect rising disproportionately with increasing dose. Moreover, the possibility of heparin-induced thrombocytopenia (HIT) exists with UFH administration. HIT is an antibody-mediated adverse reaction to heparin that can result in venous or arterial thrombosis. Hirsh J, Anand SS, Halperin JL, Fuster V. This slide shows some of the advantages and disadvantages of low-molecular weight heparin. Compared with UFH, the increased anti-Xa to anti-2A activity will inhibit thrombin generation to a greater degree. LMWH also induces tissue factor pathway inhibitor release as compared with UFH and other anticoagulants, thereby enhancing anti-clotting effects. It's not neutralized by platelet factor 4, and has less binding to all the proteins resulting in more consistent anticoagulation. With fewer long chains, there is a lower rate of heparin-induced thrombocytopenia, and there are lower fibrinogen levels. LMWH is also easier to administer because of its subcutaneous administration, and has a long history of use in clinical trials and is FDA-approved for multiple different indications. Some of the disadvantages include that it has an indirect action and therefore does not bind to clot-bound thrombin. It is less reversible because of its subcutaneous administration, which makes reversal with protamine more difficult, albeit it possible. LMWH is difficult to monitor, but monitoring is typically unnecessary. It is renally cleared which necessitates does adjustments for renal dysfunction. Hirsh J, Anand SS, Halperin JL, Fuster V. El estudio multicentrico CLARITY comparo el effecto de clopidogrel con el de placebo en el riesgo de presenatar arteria coronaria relacionado con el infarto ocluida o muerte o recurrencia de Infarto de miocardio y demostro que el uso de una dosis de carga de clopidogrel de 300mg seguida de dosis diaria de 75 mg tuvo una reduion absoluta de riesgo de 6.7% y una reduccion en la probabilidad del evento de 36%. El intervalo de confianza demostro precision en el hallazgo pues revela que la reduccion de riesgo en el grupo que usa clopidogrel puede variar de 24% a 47%.
El estudio COMMIT enrrolo 45,852 pacientes y los randomizo a terapia con clopidogrel mas asa o placebo mas asa. El estudio demostro despues de un mes de seguimiento una diferencia absoluta en el riesgo de muerte de 0.6% y una reduccion relativa de 7%, la cual alcanzo significancia estadistica. Este grafico representa la incidencia cumulativa de eventos en los dos grupos en el curso de 28 dias.
