Today’s Topic: Cypress Future Enhancements ■ The Cypress team is evaluating deployment options for Cypress to make available the new 2014 Measure Packages. ■ We will discuss the pros and cons of each of the options we've identified and seek feedback on the preferred approach. ■ The tradeoff becomes speed of 2014 Measure Packages and Cypress support for Backwards Compatibility. ■ Initial Assumption: – Cypress support for the new 2014 Measures available is the highest priority. ■ Feedback we heard last week: – Cypress support for the latest CMS QRDA IGs may be equally important. ■ Help us sort through the details… 3

Primary Tradeoffs: Faster Access vs. Backwards Compatible ■ Quickest way to deploy Cypress v2.5 with 2014 measures is to break backwards compatibility. ■ What breaks if you don’t have support for 2013 measure packages with Cypress v2.5. ■ Testing and Certification for 2013 Measure Packages would continue with the current version of Cypress, v2.4.1. Backwards Compatibility == Two Important Features – Simple upgrade path from Cypress v2.4.1 to Cypress v2.5 – Cypress v2.5 Support for BOTH 2013 & 2014 Measure Packages * Development and Test Time to Achieve Backwards Compatibility is Non-Trivial. 4

Cypress v2.5 Deployment Options ■ Pursue the deployment option that aligns best with EHR product testing and certification lifecycles. 5 Cypress v2.5 Deployment Options Option 1 Cypress v2.5 with Support for 2014 Measure Packages Only (Single EH/EP Release vs Staggered EH/EP Release) Option 2 Cypress v2.5 with Support for both 2013 & 2014 Measure Packages Option 3 Early access to just the 2014 Packages now with eventual support for backwards compatibility much later down the road.

Option 1 – Cypress v2.5 & support for 2014 Measures Only ■ Goal is to provide upgrade path from Cypress v2.4.1 to v2.5 and support only for 2014 EH/EP Measures. 6 Option 1 – Cypress v2.5 with Support for 2014 Measures. Not backward compatible for 2013 Measures. Description • Fastest way to field 2014 Measures • Field new baseline Cypress v2.5 with support for 2014 Measures Pros • Quickest path to get the 2014 measures fielded. Cons • Requires EHR Vendors to maintain two Cypress environments; o Maintain existing Cypress v2.4.1 to complete testing and certification for 2013 measures o Setup Cypress v2.5 to test and certify 2014 measures • ATLs will also need to support both environments for the duration of the grace period allowed to complete the upgrade to v2.5. Release Timeline* • One Release: Cypress v2.5 with EH/EP measure bundles in late September • Staggered Release: Cypress v2.5 with EH Measure Bundle in late August; followed by EP measure bundle by mid-November. Extend 30- day upgrade window • Extend upgrade window to accommodate policy timelines for 2013 measure specifications.

Option 2 – Cypress v2.5 & support for 2013 & 2014 Measures ■ Goal is to provide upgrade path for Cypress v2.4.1 to v2.5 with support for both 2014 EH/EP Measures. 7 Option 2 – Cypress v2.5 with Support for both 2013 & 2014 Measure Bundles Description • Simple upgrade path from Cypress v2.4.1 to v2.5. • Support for 2013 and 2014 Measure Packages on single Cypress baseline. Pros • Allows EHR vendors to test and certify both the 2013 and 2014 measures on a single Cypress baseline. Cons • Requires 2-3 months additional development time to build, test, and deploy. Release Timeline* • Release Cypress v2.5 and EH measure bundle in TBD • Release EP measure bundle, 3-4 weeks later. Extend 30-day upgrade window • Extend 30-Day upgrade window for Cypress accommodate transition from 2013 to 2014 measure specifications.

Cypress Support for QRDA IGs ■ How critical is support for QRDA IGs for EH and EP? – Does it help to expedite support for 2014 Measure Specifications, and then follow up in the fall with support for IGs. ■ What QRDA IGs do we need to support? – Draft Combined 2015 QRDA Implementation Guide – 2014 CMS QRDA Implementation Guide for Eligible Hospital Clinical Quality Measures (Effective July 1, 2014) – 2014 CMS QRDA I Implementation Guide for Eligible Professional Clinical Quality Measures – 2014 CMS QRDA III Implementation Guides for Eligible Professionals Clinical Quality Measures 9

What Breaks? ■ What problems arise if 2013 and 2014 Measure Packages need to be run in two separate configurations of Cypress. – One configuration with Cypress v2.4.1 for test and certification of 2013 Measure Packages. – A separate configuration with Cypress v2.5 for test and certification of 2014 Measure Packages. ■ How much longer will test and certification for 2013 Measure Packages continue? ■ If there isn’t a new version of Cypress available until December would that make a difference? 10
