  9. 9. 제 2 장 협착재해 2 ② 위험구역에 안전매트 설치 Installing safety mats at the hazardous areas If it is not feasible to cover the entire hazardous areas with electro- optical safeguard equipment, safety mats shall be installed to stop the press from operating when a worker accesses the hazardous area. ③ 정비·이물질 제거 등의 작업시 비상정지 스위치 작동 The emergency stop switch shall be turned on to prevent accidental operation of the press before workers access the hazardous areas to repair the press, remove foreign materials, and realign products inside the press. ④ 양수조작식 방호장치 고정 사용 Placing the dual-hand operating safeguard equipment at proper points The dual-hand operating safeguard equipment shall be placed and fixed at proper points covering the entire work performed on the press. Accident from Being Jammed by Belt Conveyor (1) 재해개요 (1) Description of accidents A worker is dead after his clothes are caught between the rotating weight roll and the moving conveyor belt as he attempts to remove foreign matter from the belt of the running belt conveyor used for stone crushing. (2) 재해발생 원인 (2) Cause of accident ① 기계가동 중에 이물질 제거 Trying to remove foreign materials while the machine is running. The accident occurred when the worker was caught between the roll and the belt as he tried to remove foreign materials. ② 안전덮개 미설치 Safety lid was not installed No safety lid was installed for the connection point between the roll ③ 전원차단 스위치 위치 부적정 and the belt. (4) Check the reverse-running prevention system on the conveyor running along the graded tracks for normal operation. (5) Check the lid or the barrier to prevent loaded articles from falling for normal conditions. (6) Wear the uniform in tidy manner to prevent it from being jammed. (7) Use an overhead bridge when passing over the conveyor. (8) Do not adjust the conveyance speed unless authorized to do so. (9) Do not load the articles on the conveyor belt unevenly. (10) Do not use the conveyor for purposes other than those specified for the conveyor. Accidents from Being Jammed in Injection Molder (1) 재해개요 (1) Description of accidents A worker tries to clear clogged mold nozzles with the gate open after disabling the linkage of the gate guard. He dies after he is caught between the molds that move due to accidental operation of the machine. Overturn on Scaffold (1) 재해개요 (1) Description of accidents A worker dies when he injures his head after he falls down a set of stairs with no handrail after losing his balance on an oily, slippery scaffolding floor during doing work. (2) Cause of accident (2) 재해발생 원인 None of slip prevention measures on the scaffolding ① 작업발판 미끄럼방지 미조치 Overturning risks resulted from failure to remove oily materials from the floor and the stairs of the scaffolding. ② 안전모 미착용 Safety helmet not worn The worker did not wear his safety helmet to protect his head in case of falling or overturning. ③ 안전난간 설치높이 부적절 Improper height of safety handrail The handrail installed around the scaffold was not high enough to prevent the probability of falling if a worker slips. (2) The outrigger shall be installed only when the body of the crane is kept horizontal. (3) Since the rated lifting weight varies depending upon work radius and the job angle, work shall be proceeded in such manner that does not exceed the rated lifting weight indicated by a weight indicator. (4) The rules of safe work shall be notified and taught. (5) A work plan shall be developed that accounts for the type and shape of cargo. (6) A supervisor shall be assigned to safe work rules. (7) The crane shall be equipped with safety equipment such as an overload prevention device, an over-lifting prevention device, and a hook release device. (8) Check safety equipment and wire-ropes for normal condition before starting work. Collision with Forklift Truck (1) 재해개요 (1) Description of accidents A worker dies after colliding with a forklift truck driven by an operator with a restricted field of vision due to a raised ladle rack on the truck’s forks as the operator transported the ladles for repair at a foundry. (2) Cause of accidents Restricted field of view by the forklift operator (2) 재해발생원인 The forklift operator shall try to secure field of view by reversing the ① 지게차 운전자 시야 미확보 truck when he cannot secure full field of view in front of the truck. This operator failed to do so. The operator shall remove the ignition key when he leaves the truck to prevent it from being operated by an unauthorized worker. ③ 지게차 안전통로 구분 Clearly mark safety path for forklift truck The paths shall clearly be distinguished between those for the forklift trucks and those for the workers to protect workers from colliding with the forklift trucks. 지게차 작업안전수칙 Rules of safe work on forklift truck (1) A forklift truck shall be operated by an assigned authorized operator only. (2) Do not stand or ride on the forks or palettes. (3) When detecting failure, stop the operation, and immediately report it to the supervisor. (4) Do not approach an operating forklift. (5) Do not load cargo exceeding the rated loading weight. (6) Always maintain stability of the forklift truck when using the truck to travel and carry cargo. (7) Do not turn the forklift truck at a steep angle. (8) Turn the power to the forklift truck off, and remove the ignition key when leaving the truck. 2-2. Accident from Collision with Resilient Wooden Piece (1) 재해개요 (1) Description of accidents A worker dies when his abdomen is impaled by a piece of wood as he pushes it with his abdomen into a gang-rip saw and wood cutter while he cuts wood for palettes. (2) Cause of accident (2) 재해발생 원인 Non-compliance with safety regulations when fault was detected on ① 기계설비 이상발견시 안전조치 미실시 the machine Always turn the power off, stop the operation and inspect the machine immediately upon detection of a problem such as a wooden piece caught between the ring saws when processing wooden materials; however, the worker forcibly pushed the wood piece into the saws. ② 비상정지 스위치 설치위치 부적절 Improper location of the emergency stop switch The emergency stop switch for for power shutoff was not located for immediate activation in an emergency. (2) Check the saw blades for any fracture, wear or damage. (3) Check the safety device for damage, normal operation or other conditions. (4) Wear protective gears such as safety glasses during work. (5) Once a saw is replaced, perform sufficient trial run before work. (6) Turn power off after completion of the work, or when leaving the machine. (7) Stop operation of the machine immediately upon detecting problem, and correct the problem before resuming operation. (8) Always keep the working environment clean and tidy by regularly removing sawdust. (9) Check the emergency stop switch for normal operation. 2-4. No lathing work standard defined ③ 선반 작업표준 미제정 The victim selected an excessively high rotating rate of 1,250 rpm of the main axle, and no work standard was available when the accident took place. (3) Measures for preventing a similar accident (3) 동종재해 예방대책 Improving work practices, establishing work standards, and ① 작업방법 개선 및 작업표준 제정 교육 training/education of them. An object for machining shall be fixed in the chuck of the lathe in such a way that the object does not protrude excessively. The conditions described above shall be included in the work standards. Workers shall be fully informed of the standards. ② 진동방지 조치 Taking vibration-prevention measures It is essential to provide and use a vibration-prevention device when machining an object fixed in the chuck. (2) A vibration-prevention device shall be used when machining a long object. (3) Hand tools shall not be kept on the main axle bed, but in a separate tool box. (6) Wear the uniform in tidy manner to prevent it from being jammed by an object for machining or a chuck. (7) Never wear cotton gloves. (8) Use a brush to removing chipped pieces.
