  • 5. Dependent Care Assistance If you are a single parent or your spouse works or is a full-time student, the amount you pay for day care for children 13 years of younger can be paid for with tax free dollars.
  • 6. Enrollment
    • There are three Ways to enroll in your FSA Plan
            • Paper form.
            • Fill out Print & Fax
            • Online with E-Signature
            • First we will do the paper form
  • 7. Go to www.access-vantageflex.com And click the forms tab
  • 8. Then click on the Employee Election Form
  • 9. Print, fill out, and fax or mail to Vantage Flex
  • 10. And secondly , the fill out, print, & fax method. From the Access-Vantage Flex Home page Click on Client Login
  • 11. At the Client Login Page click on the employee tab
  • 12. Soc. Security # Activation Code Here at the Login screen you will need to enter your Soc. Security # as your user name and the the activation code from the letter that you received as your password the first time you login. Then click the login tab
  • 13. Something easy to remember And again If you forget your PIN after you have changed it, please contact Vantage Flex at 800-871-9011. We will be happy to assist you The first time you login to your account you will be asked to create a new PIN. This will be your personal login password After you select a new PIN and confirm click the submit tab
  • 14. This is your home page. From here you can make and view elections to the Plan, view the plan documents, and create and view claims. Let’s click on Open Enrollment
