[Type text]2-01 Employment CategoriesIt is the intent of __________ Company to clarify the definitions of employment classifications so thatemployees understand their employment status and benefit eligibility. NONEXEMPT employees areentitled to overtime pay under the specific provisions of federal and state laws. An employee’s EXEMPTor NONEXEMPT classification may be changed only upon written notification by __________ Companymanagement.In addition to the above categories, each employee will belong to one other employment category:REGULAR FULL-TIME employees are those who are not in a temporary or introductory status and whoare regularly scheduled to work __________ Company’s full-time schedule. INTRODUCTORYemployees are those whose performance is being evaluated to determine whether further employment in aspecific position or with __________ Company is appropriate. Employees who satisfactorily complete theintroductory period will be notified of their new employment classification.Temporary employees retain that status unless and until notified of a change. While temporary employeesreceive all legally mandated benefits (such as workers’ compensation insurance and Social Security), theyare ineligible for all of __________ Company’s other benefit programs.__________ Company maintains a personnel file on each employee. With reasonable advance notice,employees may review their own personnel files in __________ Company’s offices and in the presence ofan individual appointed by __________ Company to maintain the files.2-04 Personal Data ChangesIt is the responsibility of each employee to promptly notify __________ Company of any changes inpersonal data. If any personal data has changed, notify the Human Resources Department.2-05 Introductory Period__________ Company uses this period to evaluate employee capabilities, work habits, and overallperformance. Either the employee or __________ Company may end the employment relationship at willat any time during or after the introductory period, with or without cause or advance notice.If __________ Company determines that the designated introductory period does not allow sufficienttime to thoroughly evaluate the employee’s performance, the introductory period may be extended for aspecified period.2-08 Employment ApplicationsIn processing employment applications, __________ Company may obtain a consumer credit report orbackground check for employment. A formal written performance evaluation will be conducted followingan employee’s introductory period. 2-10 Job Descriptions__________ Company maintains job descriptions to aid in orienting new employees to their jobs,identifying the requirements of each position, establishing hiring criteria, setting standards for employeeperformance evaluations, and establishing a basis for making reasonable accommodations for individualswith disabilities.3-01 Employee BenefitsEligible employees at __________ Company are provided a wide range of benefits. Benefits eligibility isdependent upon a variety of factors, including employee classification. [Type text]Some benefit programs require contributions from the employee, but most are fully paid by __________Company. 3-03 Vacation BenefitsVacation time off with pay is available to eligible employees to provide opportunities for rest, relaxation,and personal pursuits. Employees in the following employment classification(s) are eligible to earn anduse vacation time as described in this policy:Regular full-time employeesOnce employees enter an eligible employment classification, they begin to earn paid vacation timeaccording to the schedule. To take vacation, employees should request advance approval from theirsupervisors. Vacation time off is paid at the employee’s base pay rate at the time of vacation. As statedabove, employees are encouraged to use available paid vacation time for rest, relaxation, and personalpursuits. Upon termination of employment, employees will be paid for unused vacation time that has beenearned through the last day of work.__________ Company will grant holiday time off to all employees on the holidays listed below:__________ Company will grant paid holiday time off to all eligible employees immediately uponassignment to an eligible employment classification. Holiday pay will be calculated based on theemployee’s straight-time pay rate (as of the date of the holiday) times the number of hours the employeewould otherwise have worked on that day. Eligible employee classification(s):Regular full-time employeesIf a recognized holiday falls during an eligible employee’s paid absence (e.g., vacation, sick leave), theemployee will be ineligible for holiday pay. If eligible nonexempt employees work on a recognizedholiday, they will receive holiday pay plus wages at their straight-time rate for the hours worked on theholiday. To be eligible, employees must complete three calendar days of service in an eligibleemployment classification. These holidays must be scheduled with the prior approval of the employee’ssupervisor.__________ Company provides a comprehensive workers’ compensation insurance program at no cost toemployees, pursuant to law. Subject to applicable legal requirements, workers’ compensation insuranceprovides benefits after a short waiting period or, if the employee is hospitalized, immediately.Employees who sustain work-related injuries or illnesses should inform their supervisor immediately.3-07 Sick Leave Benefits__________ Company provides paid sick leave benefits to all eligible employees for periods oftemporary absence due to illnesses or injuries. Eligible employee classification(s):Regular full-time employeesUnused sick leave benefits will not be paid to employees while they are employed or upon termination ofemployment.__________ Company encourages employees to fulfill their civic responsibilities by participating inelections. Generally, employees are able to find time to vote either before or after their regular workschedule. Employees should request time off to vote from their supervisor at least two working days priorto the Election Day. Regular full-time employees__________ Company defines “immediate family” as the employee’s spouse, parent, child, or sibling.__________ Company encourages employees to fulfill their civic responsibilities by serving jury dutywhen required. Employees may request unpaid jury duty leave for the length of absence. If desired,employees may use any available paid time off (for example, vacation benefits).Of course, employees are expected to report for work whenever the court schedule permits.[Type text]

[Type text]Either __________ Company or the employee may request an excuse from jury duty if, in __________Company’s judgment, the employee’s absence would create serious operational difficulties.3-13 Benefits Continuation (COBRA)Some common qualifying events are resignation, termination of employment, or death of an employee; areduction in an employee’s hours or a leave of absence; an employee’s divorce or legal separation; and adependent child no longer meeting eligibility requirements.Under COBRA, the employee or beneficiary pays the full cost of coverage at __________ Company’sgroup rates plus an administration fee. __________ Company provides each eligible employee with awritten notice describing rights granted under COBRA when the employee becomes eligible for coverageunder __________ Company’s health insurance plan. Employees in the following employment classification(s) are eligible to participate inthe health insurance plan:Regular full-time employeesEligible employees may participate in the health insurance plan subject to all terms and conditions of theagreement between __________ Company and the insurance carrier.Contact the Human Resources Department for more information about health insurance benefits.All employees are paid monthly on the first day of the month. 4-05 Employment Termination resignation—voluntary employment termination initiated by an employee.__________ Company will generally schedule exit interviews at the time of employment termination.The exit interview will afford an opportunity to discuss such issues as employee benefits, conversionprivileges, repayment of outstanding debts to __________ Company, or return of __________ Company-owned property. Nothing in this policy is intended to change the company’s at-will employment policy.Since employment with __________ Company is based on mutual consent, both the employee and__________ Company have the right to terminate employment at will, with or without cause, at any time.Employees will receive their final pay in accordance with applicable state law.Employee benefits will be affected by employment termination in the following manner. Employees may requestavailable paid leave time such as unused vacation benefits.5-12 Business Travel Expenses__________ Company will reimburse employees for reasonable business travel expenses incurred whileon assignments away from the normal work location. Employees whose travel plans have been approvedshould make all travel arrangements through __________ Company’s designated travel agency.Employees are expected to limit expenses to reasonable amounts.When travel is completed, employees should submit completed travel expense reports within 30 days.Employees are responsible for the conduct and safety of their visitors. If an unauthorized individual isobserved on __________ Company’s premises, employees should immediately notify their supervisor or,if necessary, direct the individual to the main entrance.Computers, computer files, the e-mail system, and software furnished to employees are __________Company property intended for business use. __________ Company strives to maintain a workplace freeof harassment and is sensitive to the diversity of its employees. Employees in the followingemployment classifications are eligible to request medical leave as described in this policy:Regular full-time employeesSo that an employee’s return to work can be properly scheduled, an employee on medical leave isrequested to provide __________ Company with at least two weeks’ advance notice of the date theemployee intends to return to work. If an employee fails to return to work on the agreed-upon return date,__________ Company will assume that the employee has resigned.Employees in the following employment classifications are eligible to request family leave as describedin this policy:Regular full-time employeesSo that an employee’s return to work can be properly scheduled, an employee on family leave isrequested to provide __________ Company with at least two weeks’ advance notice of the date theemployee intends to return to work. If an employee fails to return to work on the agreed-upon return date,__________ Company will assume that the employee has resigned.__________ Company provides pregnancy disability leaves of absence without pay to eligible employeeswho are temporarily unable to work due to a disability related to pregnancy, childbirth, or related medicalconditions. Any employee is eligible to request pregnancy disability leave as described in this policy.[Type text]

[Type text]Employees returning from pregnancy disability leave must submit a health care provider’s verification oftheir fitness to return to work.Employees may substitute any accrued paid leave time for unpaid leave as part of the pregnancy disabilityleave period. So that an employee’s return to work can be properly scheduled, an employee on pregnancydisability leave is requested to provide __________ Company with at least two weeks’ advance notice ofthe date she intends to return to work.If an employee fails to report to work promptly at the end of the pregnancy disability leave, __________Company will assume that the employee has resigned.7-01 Employee Conduct and Work RulesTo ensure orderly operations and provide the best possible work environment, __________ Companyexpects employees to follow rules of conduct that will protect the interests and safety of all employeesand the organization.Nothing is this policy is intended to change the company’s at-will employment policy. Employment with__________ Company is at the mutual consent of __________ Company and the employee, and eitherparty may terminate that relationship at any time, with or without cause, and with or without advancenotice.While on __________ Company premises and while conducting business-related activities off__________ Company premises, no employee may use, possess, distribute, sell, or be under the influenceof alcohol or illegal drugs. • Offering employment benefits in exchange for sexual favorsTo maintain a safe and productive work environment, __________ Company expects employees to bereliable and to be punctual in reporting for scheduled work. Absenteeism and tardiness place a burden onother employees and on __________ Company. Employees are responsible for all __________ Companyproperty, materials, or written information issued to them or in their possession or control. Employeesmust return all __________ Company property immediately upon request or upon termination ofemployment. Resignation is a voluntary act initiated by the employee to terminate employment with__________ Company. Although advance notice is not required, __________ Company requests at leasttwo weeks’ written notice of resignation from nonexempt employees and two weeks’ written notice ofresignation from exempt employees.__________ Company requires the cooperation of all employees in administering this policy.__________ Company recognizes that employees may have interests in events and organizations outsidethe workplace. However, employees may not solicit or distribute literature concerning these activitiesduring working time. If thesupervisor is unavailable or the employee believes it would be inappropriate to contact that person, theemployee may present the problem to the Human Resources Department or the CEO.The employee presents the problem to the Human Resources Department if the problem is unresolved.The Human Resources Department counsels and advises the employee, assists in putting the problem inwriting, and visits with the employee’s manager(s).__________ Company supports these endeavors as long as the employees are able to meet acceptableperformance standards.
