  7. 7. 7 Choose & Book • Sandwell PCT - top rated PCT in the West Midlands; • 90% referral target; • Directly bookable appointments at local acute trust; • Advice and guidance functionality; • Directory of Services includes updated referral criteria; • Reduces DNA rates, appropriate referral; • More likely to be successful if care is integrated between primary, community and secondary care; • Map of Medicine integrated into C&B within North Staffordshire.   8. 8. 8 Map of Medicine (MoM) In the West Midlands there are various local health communities using the MoM to assist them with various transformational projects: • North Staffordshire and Stoke-on-Trent have localised a number of Musculoskeletal (MSK) and Chronic Obstructive Pulmonary Disease (COPD) pathways in order to support their ‘Fit for the Future’ Programme • A web based visual representation of evidence- based patient care journeys • Provides doctors with the information they need to make informed decisions based upon national guidance and also local processes • Accessible for patients to see through the NHS Choices website • Empowering patients to give them choice and control “The Map provides doctors with the information they need to make informed decisions based upon national guidance and also local processes.   9. 9. 9 Lorenzo Podiatry • Clinical notes: record consultations, upload photos of wounds, treatment plans and facilitates work flow. • Forms: monitor wounds, assess foot risk, record nail surgery assessments and treatments. • Clinical documents: letters back to referrers and referrals onto consultants. • Very stable and reliable, even when used out and about in patient’s homes via 3G mobile broadband.   10. 10. 10 • Calls to ambulance service are recorded through an Electronic Patient Record. • Information is sent to the ambulance which is fitted with a Panasonic Toughbook to view patient details and access to decision support software, helping to improve patient care. • The collated information is used to assists with commissioning processing, operational efficiency, an electronic database for clinical audit.   11. 11. 11 Accident and Emergency Department • Able to view Ambulance ECS patient information on the Webex Application within in the A&E department at University Hospitals Coventry and Warwickshire (UHCW); • Able to improve efficiency and safety in the A&E department: “You know what to expect and the information helps in terms of planning” Dr Ward, Consultant in Emergency Care Medicine at UHCW • Technology updates should allow for data to be assimilated into NHS Care Record Service compliant A&E records.   12. 12. 12 • Enabling clinical images to be stored electronically and viewed on screen; • Access at the touch of a button, patients can benefit from much speedier diagnoses and more informed consultations; • PACS enables the comparison of old and new patient images - measuring the effectiveness of their patients' treatment and monitor their progress; • Images are easily accessed and can be shared; • Able to view images at home, changing the way people work. “The best thing that has happened in radiology in the last 20 years” Dr Richard Wellings, Consultant Radiologist at UHCW.   13. 13. 13 Electronic Discharge Summary (EDS) • NHS Hereford has developed Electronic Discharge Summary system with all its GP practices using DOCman. • EDS significantly speeds up communication between clinicians. Able to share EDS with wider team. Community Matrons and District Nurses are also able to access the information. • Improving the quality and quantity of information sent to a patient's GP. “We are looking at expanding EDS to include Clinic Letters to improve communication with the practices.” Dr Jake Burdsall, Consultant Gastroenterologist at NHS Hereford.   14. 14. 14 Using Choose and Book to book a physiotherapy appointment • A GP or patient can book physiotherapy appointments using Choose and Book at University Hospitals Birmingham and NHS Hereford; • Offers the patient an appointment at a time and place convenient to them. MAP
