HRSA ANE Grant  This project is/was supported in part by funds from the Division of Nursing (DN), Bureaus of Health Professions (BHPr), Health Resources and Services Administration (HRSA), Department of Health and Human Services (HHS) under grant number 6D09HP25938 and title “Advancing Nursing Education: Interprofessional Collaboration addressing Multiple Chronic Conditions in Rural Communities” for $997,173.  The information or content and conclusions are those of the author and should not be construed as the official position or policy of, nor should any endorsements be inferred by the Division of Nursing, BHRr, HRSA, HHS or the U.S. Government.

Focus  Training Grant  The focus is on the implementation of an IPE model that prepares students with requisite knowledge needed to address MCC through Interprofessional Grand Round teams  The ANE-IPC-MMC proposes to advance nursing education and practice by engaging NP students and other graduate health trainees in shared interprofessional clinical and didactic experiences, to prepare them to work in rural communities.
  • 5. ANE-IPC-MCC Grant  Lead by Nursing, NP Concentration  UA Partners: CCHS, Social Work, Nutrition  Community Partners: BHSALA, UMC

  • 7. IPGR Process  Referrals from Community Partners  Patient participates for up to a year, with weekly check-in’s with NP and other team members as needed  NP students – Initial H&P  All IPGR team members meet and utilize technology to communicate with each other and the patient (patient-centered care)        Utilizing portable TM equipment  Store and Forward data collected  Enter info into EHR  Live feed facilitated by the NP on site with patient IPGR teams develop IP Plan of Care IPGR teams present to lead faculty in all professions Revise as needed Present to PCP Implement approved IP Plan of Care with patient F/U up to a year with patient, weekly basis

  • 9. Telemedicine Education  Training Grant Provides Education on TM through:  EHR – NextGen  Hands on Use of Equipment  Orientation to Technology  $227,030 TM Equipment  6 Mobile Telemedicine Packages  Videoconferencing Infrastructure  iPads with 3G/4G  Bandwidth in 4 Rural Clinics
  • 10. Vision  Students will be better prepared:  to work as a team  to practice patient-centered care  to address MCC in rural Alabama  for the future use of technology in healthcare
