    2. GASTROESOPHAGEAL REFLUX DISEASE (GERD): As the term implies, gastro-oesophageal reflux disease is reflux of the stomach contents into the lower part of the gullet. When you eat, food travels from your mouth to your stomach through a tube called the esophagus. At the lower end of the esophagus is a small ring of muscle called the lower esophageal sphincter (LES). The LES acts like a one-way valve, allowing food to pass through into the stomach. Normally, the LES closes immediately after swallowing to prevent back-up of stomach juices, which have a high acid content, into the esophagus. GERD occurs when the LES does not function properly allowing acid to flow back and burn the lower esophagus. This irritates and inflames the esophagus, causing heartburn and eventually may damage the esophagus. Cause of gastro-oesophageal reflux disease: In India and rest of the world some people are born with a naturally weak sphincter (LES). For others, however, fatty and spicy foods, certain types of medication, tight clothing, smoking, drinking alcohol, vigorous exercise or changes in body position (bending over or lying down) may cause the LES to relax, causing reflux. A hiatal hernia (a common term for GERD) may be present in many patients who suffer from GERD, but may not cause symptoms of heartburn. Treatment available for gastro-oesophageal reflux: Generally GERD is treated in three different ways: 1. Lifestyle changes: The most important lifestyle change to improve the symptoms of reflux is losing weight. If you are overweight, there is often a critical weight. Below this the symptoms of reflux will improve dramatically, above it, reflux will be prominent. Reducing smoking and alcohol consumption will also be helpful. Changing eating habits will also improve symptoms. It is important to have regular meals and to have the last meal several hours before going to bed. 2. Medical treatment: If symptoms persist after these life style changes then medical treatment is another option. 2.1 Drug therapy: Drug therapy is usually very successful at improving the symptom of heartburn. Antacids neutralise the stomach acidity and will relieve relatively mild symptoms. If these fail then stronger prescription drugs may be necessary. These are known as proton pump inhibitors.
    3. Antacids neutralize stomach acids and over-the-counter medications reduce the amount of stomach acid produced. Both may be effective in relieving symptoms. Prescription drugs may be more effective in healing irritation of the esophagus and relieving symptoms. This therapy needs to be discussed with your surgeon. 2.2 Esophageal Dilation: If you have a narrowing in your esophagus or start to have trouble swallowing, your physician may recommend you have one or several dilatation procedures to enlarge your esophagus. Flexible rubber tubes (bougies) of increasingly larger sizes are placed into your esophagus to make it wider. 3.
