that this loan is for educational purposes only and that this application, as well as the Promissory Note,
which I will sign after the loan has been approved, will serve as my authorization for my loan proceeds
to be automatically credited to the student account of the student indicated above. I understand that
funds borrowed for expenses above direct college costs will be paid by the college directly to me.
In addition, I provide permission to transmit the above information
to the Direct Loan servicer for the U.S. Department of Education, and that the servicer has my
permission to evaluate my credit for purposes of this loan. It is fully understood that my credit report
will be evaluated by the servicer for the U.S. Department of Education, and that the credit report will
not be provided to the institution and will not become a part of the student's confidential file. I
understand that both the institution and myself will be notified of approval or denial, and that any
reasons for a denial will be sent only to me.
