    • Ms.Metilda Robin,Deepak Agarwal Nurse Informatics JPNATC,AIIMS
    • BACKGROUND  Computerisation of clinics in public funded (Government) hospitals has been cursory at best.
    • Background  Typically a doctor spends less than 2 minutes on a patient, during which time he has to listen to patients complaints, formulate a diagnosis & treatment plan and write down the notes in the patients clinic card as well as in the hospital record (EMR etc).
    • CLINICIANS WISHLIST FOR CLINICS  See Scheduled Appointments  Look up previous visits, labs, PACS images  Avoid typing  Take Clinical photographs, store with patients record & retrieve at leisure  Do all this in less than 60 seconds!
    •  We tried to enter in EMR system in OPD for nearly two years(2011,2012)  But we got 0% compliance.
    • Solution Provider: Automated Tablet based Clinic(OPD) with messaging system and web portal http://aiims.mcura.com anywhere anytime access for doctors
    • 1. Patient calls call centre for appointment 2. Call centre checks Dr. schedule and allocates a slot to patient Appoi ntmen t details 3. mCURA platform shows appointment list to doctor when doctor logs in the tablet app 4. Doctor retrieves patient record and adds advice/take pics/ reviews radiology images and assign next visit time 5. Reminder sent to patient mobile 72 hrs – 48 hrs to fix appointment for review
    • •Appointment schedule and sms based token number messaging •OPD Notes that includes Patient’s wound pictures •Integrated PACS where Dr. can mark their comments on a specific image slice •Drug index access so to select drugs and review other attributes of drugs formula •OPD notes on their own handwriting using stylus •Next visit entry and sms reminder to patient’s mobile
    •  Started in Jan 2013  Test trial given for 2 month with continuous training  Feed back was taken and added the requirements.  Implemented live from march 2013  Compliance rate has increased in last three months.  In august we had 50% usage of tablets in OPD.  Patients are also satisfied with this as a copy remains in the hospital also
    •  A change in the existing system of manual notes to the tablet usage was very tough.  Continuous training and motivation was done by NIS.  More over in a busy OPD any time lag was not acceptable to the physician.  Network should be uninterrupted to get good workflow.
    •  No OPD records were available to the Hospital previously.  Now we have a computerized record in which the we can see the records by web interface from anywhere in hospital.  In one pt file we can attach the notes ,x-ray images, wound images which can be revived again in next visit.  It is helpful to get a statistical data of the outpatients which helps in research purpose.  It is now web interfaced so any doctor can access the record of his/her opd visit in their room itself
