    • Change Readiness Assessment Instruction: This questionnaire is aimed at determining your past experience regarding changes that were made in your organization, and also how you perceive the new project. You are given a series of questions that you need to rate, according to your experience, and indicating whether you agree or disagree with the respective statements. Remember, you are rating your own experience and your response is anonymous Scale: For this exercise we request you to answer every question according to following scale: 1 Strongly Disagree 2 Disagree Somewhat 3 Neutral 4 Agree Somewhat 5 Strongly Agree 1.
