  • 1. Welcome!
  • 2. We are Clara’s House!
  • 3. Mission Clara’s House  addresses the health needs of the poor as a manifestation of the spiritual beliefs of the participants cherishing diversity of cultures and religious beliefs.
  • 4. Vision The clinic provides primary health care and education for those economically disadvantaged among us.  Nurse Practitioners operate the clinic in collaboration with volunteer MDs, NPs, RNs, and other health care professionals. They further extend care to those who do not have access to health care and are not aware of appropriate resources. The clinic staff works in coordination with existing services in the community and seeks to promote health maintenance through education, assessment and preventative health care. The staff coordinates ongoing care with professionals, agencies, schools, families, and  the larger community.
  • 5. A primary care clinic providing comprehensive health care Clara’s House is:
    • An all-volunteer, unpaid staff
    • Does not charge patients fees for medical services
    • Asks partner agencies who refer patients to provide community support
  • 6. New Process
    • MOU agreements renewable yearly
    • New referral process online (Fax cutoff 5/1/2011)
    • Notification to partner of acceptance or non-acceptance of referral
  • 7. Partner Agency 3R’s:
    • Refer uninsured, low income individuals only and for primary care only (not eligible for county, state, or federal health care benefits)
    • Responsibility for social services needs of referred who are accepted as patients, including helping to find medical specialists and advocacy with other agencies
    • Referral is a PDF file sent by email or online
  • 8. Clara’s House will:
    • Respond in a timely manner to referrals on acceptance or non-acceptance
    • Schedule initial appointments as soon as possible
    • Treat patients providing primary comprehensive health care
  • 9. Primary health care is : Not urgent care: flu symptoms, asthma attack, UTI Not emergency room care: heart attack, broken bones, stroke Not specialty medical care: surgery, cardiac, psychiatric care, x-ray
  • 10. Urgent care is: "Urgent care is the delivery of ambulatory care in a facility dedicated to the delivery of medical care outside of a hospital emergency department, usually on an unscheduled, walk-in basis. Urgent care centers are primarily used to treat patients who have an injury or illness that requires immediate care but is not serious enough to warrant a visit to an emergency room. Often urgent care centers are not open on a continuous basis, unlike a hospital emergency room which would be open at all times.” -- from Wikipedia

    • Each agency shall be emailed an electronic referral form which allows the MOU partner to file referrals online or to email them.
    • Once the referral is received, Clara's House will acknowledge receipt and will indicate whether or not the referral is accepted.
    • If a referral for a patient is accepted, Clara's House agrees to become the primary health care provider for the patient.
    • Once a referral is accepted, the agency or the patient may phone the clinic to make an appointment.
    • If patient signs an authorization release, specifics of the case may be discussed with the referring agency. Referring agency may be contacted by Clara's House to provide community support services.
    Referral Process

    • Patient is NOT in need of urgent or emergency care.
    • Patient applied/will apply for county or state health care if eligible.
    • Referral form is completed and sent to the clinic.
    • Once accepted and an appointment is made, patient is asked to bring all medications and information about previous medical records to the first visit.
    • A new patient is asked to arrive 15 minutes early for a first appointment in order to complete paperwork.
