It’s your responsibility to request your medical record
2. You can ask the doctor to explain it to you, make changes and/or re-write illegible items
What you say to your doctor impacts the quality of the information in your record (and your care)
Your medical record generally can’t be shared with others without your consent (numerous exceptions)
5. It’s your responsibility to make sure new doctors get copies of your old records
