An impression of the arch is made with a temporary partial denture in place.

A vaccuform is made of the cast with the partial denture in place holes are drilled through the vaccuform into the base of the cast to repeat the position of the teeth.

3 mm X 30 mm Guide posts were placed into the holes drilled in the cast

The anterior view demonstrates the proposed angle of the proposed osteotomies.

Two guide sleeves were placed onto the post and the body of the template is fabricated with Triad® Gel extended over several of the adjacent teeth distal to the edentulous area.

A cone bean 3D X-ray is taken to show the alignment of the guide sleeve in the # 26 position Invivo 5

Mesio-distally the alignment is too close to the adjacent tooth, Therefore it will be moved 0.5 mm away from the adjacent cuspid.

The bucco-lingual alignment is acceptable as is.

If the orientation line is not perpendicular to the buccal surface of the cast resulting in the absence of the adjacent tooth indicating an error reading has been made.

A better position of the implant placement in the # 23 position will be made if the guide sleeve is moved lingually 1 mm.

Two 1 mm offset guide posts are used to reposition the long axis of the guide sleeves in the #23 position 1 mm lingually and the offset guide post in the #26 position is used to move the guide sleeve 1 mm distally.

Two 1 mm offset guide posts are used to move the osteotomy in the #23 position 1 mm lingually and the position of the guide post in the #26 position was moved 1 mm mesially.

Open guide sleeves are placed on the guide posts

Clear picture of the surgical site.

A crestal incision was made and the template is placed. Two 2 mm pilot drills with a 2.9 mm guide sleeve are used to make the initial opening in the alveolar crest using 3.4 mm open guide sleeves.

A 2.5 mm drill on the right a 2.7 mm drill on the left are used to enlarge t he osteotomies centered with in the 3.4 mm open guide sleeves.

A 3.2 mm and a 3.35 mm Astra drill are placed in the 3.4 mm guide sleeves.

The two implant carriers are inserted through the open guide sleeves to demonstrate the accuracy. Unfortunately, when the template was replaced for a photo the guide sleeve on the right was not seated on the teeth.

The two implants were placed and the surgical site is ready for closure.

The final X-ray taken with the surgical guide in place demonstrates the precise implant placement with the surgical template.
