Mandibular Bicuspid Placement requiring SINGLE BEND corrections in one plane NOZ

Start with a wax up. With a marker draw a line to indicate a proposed osteotomy axis.

Remove the wax up and place a 3 mm guide post in the hole.

Place a 3 mm ceramic guide sleeve over the post.

Place a radio-opaque/ceramic guide sleeve over the guide post. Capture the bracket with a small amount of Triad® Gel.

Place the template and take a periapical X-ray. A 12 °change is required to avoid the mandibular nerve.

Tighten the set screw.

Steps 3 & 4 Place the stylus over the guide post. If the guide post is straight the point will register on zero degrees. If the guide post is not straight discard the guide post. The bottom of the stylus should be in contact with the top of the V block unless the guide post is offset.

The template was remade with a 10 ° bend and the osteotomy started with a 2 mm drill. A direction indicator is placed to verify the path of insertion.

Final implant placement.

The 1 st correction bend is always made by rotating the stylus to the right or left.
In this case to 12° indicated by the angle observed in the tangential view of the cone beam X-ray. You may need to ease the point of the stylus 3 or 4 degrees beyond the point of the desired degree to compensate for the memory of the
metal so that the point of the stylus rests on the chosen degree.

Guide Right™ GENERATION II BENDING TOOL SINGLE BEND review Step 2 Locate 3/32” hole in the center of the v-cut and place the bottom half of the guide post into the hole. Tighten the set screw. Step 1 Place bending tool plate on a secure flat surface with the degree increments at the top & the stainless steel bar with the v-cut at the bottom. S tep 3 Locate the hole in the bottom of the stylus that you will use that will fit over the top half of the guide post (3.0 mm, 4.0 mm or 5.0 mm). Step 5 Using the stylus as a lever, bend the guide post to the degree of angle of correction. You may need to ease the point of the stylus beyond the point of the desired degree. Step 6 Loosen screw and remove guide post and the stylus to find the guide post bent to the desired angle. Step 4 Fit the stylus over the guide post securely with the point directed at zero degrees and the bottom of the stylus in contact with the V block.

After the first bend is complete rotate the stylus with post back to the 0° position. The stylus will lift up off the face of the bending plate. Slide the stylus support bar down to make contact with the surface of the stylus. The wing screws must be tightened to prevent movement of the support bar The 2 nd bend can now be made in either direction according to the needs indicated by the X-ray.
Step 4 a

The second bend is made by rotating the stylus to the right or left against the
stylus support bar to 15° as indicated in the cross sectional view of the X-ray.
The support bar must first be secured with the lateral thumb screws.

GUIDE RIGHT™ GENERATION II BENDING TOOL COMPOUND BEND review Step 1 Position a straight or offset guide post in the bending plate, tightening the set screw against one of the flat surfaces on the lower half of the guide post. Step 4 The 2 nd bend in the second plane is made after rotating the guide post up. away from the surface of the bending plate to register the stylus point back at 0 degrees. Step 5 Slide the stylus support bar down under the stylus until it supports the stylus. Tighten the side screws before making the second bend. Step 7 Remove the stylus and place the guide post back in the cast with the appropriate side indicated by a mark facing the buccal or lingual surface. Be sure the post is in the correct position . If the post needs to be corrected by a linear movement an offset guide post can be used. Off sets available in the 3 mm guide post: 0.5,1,1.5, 2.0 ,or 3.0 mm. Step 3 The set screw is loosened and the guide post is rotated 90 ° next flat surface. Step 2 The 1 st bend can be made to the right or left direction. Step 6 The second bend can be made in either direction according to the x-ray.

This is a case with a narrow ridge & a lingual undercut in the molar region. 3/32” holes have been drilled in the cast after marking the distances for the teeth on the ridge.

Three straight guide posts are used to make a template for cone beam X-ray only.

Three ceramic guide sleeves are used to make the scanning template to minimize the scatter observed with cone beam X-rays. However metal sleeves can also be used.

The X-ray shows the three implant sites in the panoramic view. Three virtual 3.5 mm diameter implants have been placed in positions # 18,19 and 20. Each site will have to be corrected in order to place the implants because the mandible has a significant lingual inclination with a lingual undercut.

Site #20 Site # 20 is evaluated and corrected first. The X-ray shows a compound bend will be necessary to correct the implant placement: 12° mesio-distally and 15° bucco-lingually Angles are measured with software or a protractor and are used to make the correction. cross sectional view tangential view

Three magnetic guide posts are bent to make the corrections.
A new template is made with open guide sleeves.

The three corrected magnetic guide posts are placed back in the cast.

The 3 mm open guide sleeves have been chosen for the final template to allow good visibility of the drill depth markings .

Triad® Gel is added to complete the fabrication of the corrected template.
