UK College of Dentistry 6/21/2006 STEP 1 – Treatment Tab Select the Treatment Tab Do not post treatment here CLICK TREATMENT TAB, to the right of the General tab and upper area of the Transaction window. Note: Although it is possible to enter planned treatment from this screen, we will be entering all planned treatment from the Dental Chart. axiUm Training Materials: Treatment Posting 4

UK College of Dentistry 6/21/2006 STEP 2 – New Claim Click the New Claim Button Select the type of claim here CLICK THE NEW CLAIM BUTTON to add new treatments (even if the patient has no insurance). If the claim is a medical claim ensure that the “Medical” button is checked. axiUm Training Materials: Treatment Posting 5

UK College of Dentistry 6/21/2006 Enter Claim Information Ensure information is accurate If the Practice, Date, Billing ID, or Facility information is incorrect, choose the correct information from the drop-down menus. Billing ID: - If the provider is a student, you MUST choose “&&STUDENT” from the drop-down menu - If the provider is a resident, you MUST choose “&&RESIDENT” from the drop-down menu - If the provider is a hygienist, you MUST choose “&&HYGIENIST” from the drop-down menu - If the provider is a faculty, you MUST choose the faculty member’s provider number (Example: F1234) - If the patient has UKDC insurance, use the appropriate UKDC fee schedule for the given provider type (UKDCFAC, UKDCRES, or UKDCSTU). If the provider is a hygienist, use UKDCFAC. Note that for students, residents, or hygienists, entering the Billing ID in this manner will ensure that the faculty member’s information appears on insurance claim forms. Facility: - Please select the appropriate Facility from the drop-down menu. This field MUST be accurate for claim information to be correct. axiUm Training Materials: Treatment Posting 6

UK College of Dentistry 6/21/2006 Enter Instructor (Attending) Information Select the Instructor (Attending Doctor) When you click this button, the system will present a decision box asking if you want to continue searching. Click the Yes button. Enter the last name of the Instructor and click on the Last Name Column heading. Double click on the appropriate name. axiUm Training Materials: Treatment Posting 7
  8. 8. UK College of Dentistry 6/21/2006 STEP 3 – Code Field Make sure to put in the actual date of service, not always the current date This is the “More” Button 1. IN THE CODE FIELD, click the “More” three-dot box and select the appropriate ADA or CPT code from the list. 2. IN THE TREATMENT DATE FIELD, make sure to put in the actual date of service, not always the current date. axiUm Training Materials: Treatment Posting 8

UK College of Dentistry 6/21/2006 Procedure Code List Select Type of Code here Search by Code or Procedure Description Select Category (Discipline) here In the TYPE OF CODE field, ensure that the appropriate type of code (ADA or CPT) is selected. Select the specific CATEGORY from the drop-down menu, OR select the All Categories option, then Search On the specific code or description. NOTE: If you are searching for a Dental Code, you MUST type the letter “D” before the code number. (Example D0120) Also, you MUST type the letter “M” before a Medical code. UK College of Dentistry 6/21/2006 STEP 4 - Site, Surface, Unit Enter site(s) and surface(s) here Enter number of Units here, if multiple units for a CPT code A Site may be a Tooth, an Arch or a Quadrant. If a Site is required, enter Site in appropriate field. If the Site is a Tooth and the CDT code requires a surface, you will enter it in the Surface field. Arch = UA or LA Quadrant = UL, UR, LL, LR Surface = M, O, D, L, B, F, I If multiple units of the same CPT code were given, enter quantity number in the Units field. Ensure the Total, Patient and/or Insurance fields have fees, UNLESS the code is an “unspecified” or “by report” code, and enter a fee in the Total field. - If the fees do not appear, you may have forgotten to select a billing ID in the Claim area. axiUm Training Materials: Treatment Posting 10

UK College of Dentistry 6/21/2006 STEP 5 - Select a Provider Click the “More” button or enter the provider number directly Click the “More” button to search for the provider who rendered treatment Note that if you know the provider number, you may enter it directly in the Provider field. axiUm Training Materials: Treatment Posting 11

UK College of Dentistry 6/21/2006 STEP 7 – Status & Transaction Location The Status will default to “Complete” Choose the location where the treatment was provided In the STATUS field, select the procedure as either In Process or Complete. (Note: The Status will default to “Complete”.) In the TRX Location field, use the drop-down box to select the location where treatment was provided.   13. 13. UK College of Dentistry 6/21/2006 STEP 8 - Discipline A Discipline MUST be present before Treatment entry can be completed. This field SHOULD default. In the DISCIPLINE field, use the drop-down box to select the discipline that provided the treatment. AxiUm will not allow the Treatment to be added unless a Discipline is selected. This field SHOULD default. If it does not, please contact an axiUm advisor. axiUm Training Materials: Treatment Posting 13
  14. 14. UK College of Dentistry 6/21/2006 STEP 9 – Attending Doctor Select the Attending doctor here. Select the attending provider from the ATTENDING DROP-DOWN LIST. After clicking the drop-down, you may enter the first letter of the Attending Doctors last name to more quickly find the correct Attending Doctor. axiUm Training Materials: Treatment Posting 14

UK College of Dentistry 6/21/2006 STEP 11 - Additional Medical Information Click to enter additional information for Medical procedures (ICD-9 codes, anesthesia minutes, etc.) If the Code is a medical/CPT code, click the Medical button to enter ICD-9, place of service and other relevant information. axiUm Training Materials: Treatment Posting 16

  20. 20. UK College of Dentistry 6/21/2006 STEP 12-New Treatment (same provider) Click “New” (NOT “New Claim”) to enter additional treatment by the same provider for the same claim. If additional procedures were performed by the same provider, click the NEW button (in the Treatment area) to add additional treatments to the same claim.
