Research Focus: Image-Guided Surgery My long-term interest in Image-guided surgery, was originally developed within the Brain Imaging Centre at the Montreal Neurological Institute at McGill. The focus of this work was to provide to neurosurgeons with tools to enable them to approach targets within the brain based on detailed information provided via imaging modalities such as anatomical, vascular and functional MRI, CT, DSA, PET, Ultrasound and electrophysiological information. The key to this work is to enhance the ability of the surgeons to follow their progress during procedures by using a hand held probe, or a surgical instrument, whose position is tracked by a computer. The position and orientation of the probe is then represented within a three dimensional image of the brain at all times during the operation. The original neurosurgical goals of this work will continue at the Robarts, often in collaboration with the MNI, but will also expand to include the application of image-guidance to other parts of the body. In particular, many of the techniques already developed will have immediate impact on interventional radiology, where endoscopic images may be combined with images from traditional imaging modalities to both increase the precision of the endoscopically guided procedure, and reduce its invasiveness. My research has always been, and will continue to be, clinically oriented, with clinicians playing important roles in all aspects of my work. The objectives of the research in my laboratory are being addressed via the following currently active project areas. Stereoscopic visualization in the operating room The 3-D image-based model of the brain is presented to the surgeon in a realistic form through the use of stereoscopic displays. In this manner the surgeon is able to more accurately localize the target and plan the trajectory of approach while avoiding sensitive structures. Integration of cortical surface images with 3-D models The real world of the operating room (via stereoscopic video images) and the digital MR image of the patient’s brain, are merged in order to allow the surgeon to visualize the target (tumour, cyst, epilepsy focus etc) within the patient's head prior to surgery. A similar procedure using a laser scanner to image the cortical surface is also being used to track the shift of the brain during open craniotomies. Correlation of x-ray and MR angiography X-ray angiography has been a gold standard for the visualization of cerebral vasculature. Now, however, MRI presents alternative means for imaging these vessels. In order to understand the limitations of using MR in place of X-rays, we have developed a stereoscopically based 3-D correlative imaging environment within which to objectively compare the two modalities. Accuracy of image-guided neurosurgery. There are many aspects of an image-guided surgery system that can potentially introduce errors into the system. Since one of the objectives of IGNS is to achieve an accuracy and precision of better than 1mm (particularly for functional stimulation, ablation and tissue implants), we are - 10 -
  11. 11. Peters, Terry November 2001 expending a great deal of effort to identify sources of error and to propose means of eliminating them. Neurosurgical guidance with 3-D Ultrasound Prior to my move to London, I had begun collaboration with the advanced imaging Research Laboratories at the Robarts Research Institute to develop a real-time 3-D ultrasound system that combines 3-D MRI with 3-D ultrasound, to track morphological changes of tissue during surgery. This project is now beginning its second phase, where the MRI image will be updated in real time on the basis of information gathered by the 3-D ultrasound system. Optimal Imaging for IGNS In order to achieve the stated goals of accuracy for IGNS, we are actively developing 3-D MR imaging techniques that are capable of high resolution and high geometrical accuracy, as well as evaluating the optimal means of displaying and interacting with the images. Integration of Brain Atlases with 3-D MRI Even the highest quality MR images fail to demonstrate some of the fine detail and structures necessary for the surgeon to perform certain procedures, for example, thalamotomy and pallidotomy. On the other hand, detailed atlases of these structures exist, and they may be merged, with a non-linear warping procedure, with the patient’s MR images to provide additional guides and landmarks during surgery.
