Liver Cirrhosis β€’ Largest gland in the body β€’ 4 lobes β€’ Produced bile β€’ Contains bile salts, pigments, phospholipids, cholesterol and a variety of electrolytes β€’ Definition: β€’ Diffuse disorder of liver characterised by; β€’ Complete loss of normal architecture, β€’ Replaced by extensive fibrosis with, β€’ Regenerating parenchymal nodules οƒ˜ A chronic progressive disease of the liver characterized by diffused damage to cells with fibrosis and nodular regeneration οƒ˜ Repeated destruction of hepatic cells causes the formation of scar tissues οƒ˜ Cirrhosis is common end result of many chronic liver disorders. οƒ˜ Diffuse scarring of liver – follows hepatocellular necrosis of hepatitis. οƒ˜ Inflammation οƒ˜ Loss of normal architecture & function. οƒ˜ Major types of Cirrhosis οƒ˜ Laennec Cirrhosis οƒ˜ Post necrotic οƒ˜ Biliary οƒ˜ Cardiac οƒ˜ Etiology of Cirrhosis οƒ˜ Alcoholic liver disease 60-70% οƒ˜ Viral hepatitis 10% οƒ˜ Biliary disease 5-10% οƒ˜ Primary hemochromatosis 5% οƒ˜ Cryptogenic cirrhosis 10-15% β€’ Pathogenesis: β€’ Hepatocyte injury leading to necrosis. β€’ Alcohol, virus, drugs, toxins, genetic etc.. β€’ Chronic inflammation - (hepatitis). β€’ Bridging fibrosis. β€’ Regeneration of remaining hepatocytes Proliferate as round nodules. β€’ Loss of vascular arrangement results in regenerating hepatocytes ineffective. β€’ Assessment β€’ Anorexia and wt. loss β€’ Early morning nausea and vomiting (with blood) β€’ Flatulence and changes in bowel habits
    2. β€’ Emaciation β€’ fatigue οƒ˜ Jaundice οƒ˜ Abdominal pain and tenderness οƒ˜ Ascites οƒ˜ Peripheral edema οƒ˜ Dry skin and rashes οƒ˜ Petechiae οƒ˜ ecchymosis οƒ˜ Spider angiomas (nose, cheeks, upper thorax and shoulders) οƒ˜ Hepatomegaly οƒ˜ Protruding umbilicus οƒ˜ Dilated abdominal veins οƒ˜ Fector hepaticus οƒ˜ Asterixis οƒ˜ delirium Males (increase estrogen) οƒ˜ Gynecomastia οƒ˜ Impotence οƒ˜ Fall of body hair οƒ˜ Atrophy of testicles Females (increase androgren) οƒ˜ Hirsutism οƒ˜ Acne οƒ˜ Deepening of voice οƒ˜ Increase virilism

(Viral hepatitis, Alcohol, congenital, drugs, toxins & Idiopathic) οƒ˜ Characterised by diffuse loss of architecture. οƒ˜ Fibrous bands & regenerating nodules distort and abstruct blood flow. (inefficient function) οƒ˜ Hepatocellular insufficiency & portal hypertension. οƒ˜ Shrunken, scarred liver, ascitis, spleenomegaly, liver failure, CNS toxicity.
