Solution OverviewDescription: • Patient Inquiry® is a patient survey system used to collect functional outcomes and satisfaction data for outpatient rehabilitation in approximately 1800 facilities in virtually every US state. It is a hosted, web-based solution. • The Patient Inquiry Allscripts Connector will be a .NET application running behind the customer firewall and will establish a secure connection to the hosted Patient Inquiry® system. • The connector will provide patient data to the Patient Inquiry® system from the Allscripts system, and receive patient survey reports and data from Patient Inquiry® and send it to the Allscripts system. • The connector will introduce enormous efficiencies saving valuable staff and therapist time in collecting and managing the outcomes collection process.
  • 3. Solution Overview (continued)Healthcare/Business problem addressed: • Pay for performance reimbursement models are the future for healthcare providers. In outpatient rehabilitation, the future is now. In November 2012, CMS ruled (CMS-1590FC) that outpatient therapy providers must measure and report outcomes on claims in 2013, or claims will be rejected beginning July 1, 2013. Administering, collecting, and reporting on claims consistently and correctly will be a time-consuming and tedious challenge for outpatient therapy directors, therapists, and staff. However, Patient Inquiry® is a very efficient system for collecting outcomes. In fact, in the rule, CMS referred to Patient Inquiry® 9 times, and noted it is a model system for therapy providers to collect outcomes. • The Patient Inquiry Allscripts Connector will introduce even more efficiency and consistency by automating the process of getting the measurement data into the patient’s electronic record (required by the rule), and getting the correct G-Codes and severity modifiers on the claim. • Beyond the efficiencies gained in compliance, the time and money the provider invests in our approach will elevate the organization into a continuous improvement paradigm, ready-to-go for pay-for-performance.The Patient Inquiry Allscripts Connector application developed addresses the category of “Innovative approaches to addressing value-based care imperatives.”
  • 4. Usability and DesignPatient Centric System • Patients may take surveys from home, or in the facility, on virtually any PC or tablet with a web browser. Questions are multiple choice, with easy, large font, touch screen compatible answers. The system issues questions one-at-a-time, and employs a computer adaptive testing software model and item response theory algorithms to eliminate unnecessary survey questions. So instead of the knee patient answering a 20 question Lower Extremity Functional Scale paper survey, he only must answer 4-7 questions in order for the system to calculate an accurate score. Upon survey completion, on average after 3 minutes, outcome measures and correct G-Codes and severity modifiers are automatically assigned and calculated. The Patient Inquiry Allscripts Connector application will then automatically store the patient survey outcome data and appropriate claims data in the Allscripts electronic medical record system and billing system. • The automation the Patient Inquiry Allscripts Connector application will provide will drastically reduce the time required by management, therapists, and staff to devote to compliance. It will also reduce error and its accompanying risk.

Development StageDevelopment stage as of the Phase 1 Submission Date: • The Patient Inquiry Allscripts Connector is an Idea. • Patient Inquiry® and its web services API is in production.Additional information on the degree of development: • While the Patient Inquiry Allscripts Connector is an idea at this point, it is based on two production applications: o A robust .NET web services API in production at share.patient-inquiry.com, and o A behind-the-firewall application FOTO has in production to connect to local and network computers at the facility to automatically print, or automatically store to pdf, patient specific reports for therapists to review and keep as part of the patient record. • Development should be able to leverage the production level systems and our extensive experience with our behind-the-firewall paradigm to produce a production-ready Patient Inquiry Allscripts Connector application in a reasonable timeframe.
  • 6. Integration Description• The Patient Inquiry Allscripts Connector will share information with the Allscripts Sunrise application. This provides for seamless connection between the functional data from outpatient therapy patients and the core acute care data system.• FindPatient – to pull patient data from AllScripts and send to Patient Inquiry®.• GetOrders and/or GetProblems – to retrieve and present to clinicians in Patient Inquiry® allowing them to establish a link between the functional assessment they are performing and the problem recorded.• GetPatientVisits – will be queried and recorded in Patient Inquiry® to assure consistency and provide for streamlining of discharges in Patient Inquiry.• Reports – use the Attachment object to send documents along with the needed specific data for bill preparation to Allscripts. Since the clinician provided the link earlier in the process these are sent in tied to the correct patient and problem.
  • 7. Go to Market Plans• Beta with common Allscripts and FOTO customers to work out issues, develop case studies, and secure marketable customer recommendations and quotes. (2013 Q2 and Q3)• Production product pre-marketed to FOTO’s existing customers for quick penetration once ready for production.• Target Allscripts customers who treat Medicare patients for outpatient rehabilitation, physical or occupational therapy.• SaaS model o Price of Patient Inquiry® and Outcomes Manager™ is $75 per month for the first clinician at an outpatient clinic, and $25 per month for each additional clinician. There are setup fees of $250 per clinic. o Price of the Patient Inquiry Allscripts Connector will be an additional $80 per clinic per month, or an additional $5 per clinician per month, whichever is more.
