  • Solution Overview  High Level Solution Description: • Beacon was developed to help medical practices better manage and care for patients tied to accountable care organizations (ACO).  Healthcare/Business problem addressed: • There is a great need in the marketplace for an integrated software solution that helps providers at the point of care manage ACO patient populations.   • Usability and Design Beacon will be integrated into AllScripts and the staff upon rooming a patient will immediately be able to see if the patient has issues that need to be addressed. When the provider meets with the patient he/she can review problem areas and make recommendations. Providers can create lists on the fly of high and moderate risk patients and actions can be taken to bring the patient back in for a follow-up. We want providers and staff to take a more proactive approach to patient care.   • Video Demonstration http://vimeo.com/user16235430/beacon The video highlights the problem doctors face with the growing amount of information they must siphon through as more patient data becomes available to them. The introduction of Accountable Care Organizations only exacerbates the problem.   • Why this solution be selected! Most providers and clinicians have no idea where a patient goes when they leave the office. With the advent of ACO programs, providers now have this data, but don’t know what to do with it. We feed this data back to providers along with quality information through an integrated solution with the AllScripts eMR. We feel more and more patients will be tied to ACO’s and systems will need to be put in place to manage these patients.
