Eligibility/ Qualifying criteria • Must come from the following: – Province – Municipality – Chartered City • Formally organized/ established/created office – With separate staff and budget – With resolution, ordinance, EO – With office/physical structure • With Cooperative Development Officer duly appointed/ designated (full time) • Must have cooperative development programs • Must be endorsed by at least one cooperative in the area

List of the Officers and staff certified by LGU’s personnel officer including the status of appointment 3. Short (annual) and long-term (3-5 years) Development Plans and programs for co-op development 4. List of Policy/ local issuances affecting cooperatives 5. Approved Budget & Work and financial Plan of the Unit 6. Annual Accomplishment Reports attested by the Mayor/Governor
  6. 6. DOCUMENTARY REQUIREMENTS 7. Detailed summary report of trainings, mentoring and consultancy attested by the Mayor showing the ff: A. Training - Title of training - No. of cooperative participants - Amount spent - Date of training B. Mentoring - Nature of mentoring - Name of cooperative beneficiary C. Consultancy - Nature of consultancy - Name of cooperative beneficiary
  7. 7. DOCUMENTARY REQUIREMENTS 8. List of cooperatives assisted in terms of compliance to legal requirements (e.g. CTE, etc.) 9. Proof of marketing support to cooperatives- pictures, approved training design, activity report 10. List of cooperative organized 11. Proof for cooperative promotions - Flyers, newsletter, primers, etc. - Pictures of tarpaulin - Website - Evidence of radio and TV programs 12. Disbursement report of cooperative for financial assistance 13. Proof of CDA-LGU Partnership(i.e. MOA) 14. List of awards/ recognitions received by the cooperative including the sponsor of the awards and the year received (provincial/ city/ regional/national levels) for the past 3 years.
