Theplan includes the following benefit programs:Medical/Dental/Prescription/Vision - Three nationalPPO medical coverage options and two PPO dentaloptions are available. Kaiser HMO is available toemployees in California. Prescription and vision coverageis provided based on elected medical coverage. Premiumcontribution amounts vary according to salary range.Flexible Spending Account - Health and Dependent Careaccounts are available. Commuter accounts are alsoavailable, based on location.Disability/Life Insurance - Short-Term Disabilitycoverage is provided at no cost. Upon approval, a salarycontinuation schedule of 60–100% current salary isoffered. This benefit provides one week of paid leaveand up to a $4,000 reimbursement of eligible adoptionexpenses.401(k) Retirement Plan - Constellation offers a 401(k) retirement plan with a 3% match to allparticipating employees.Paid Time Off (PTO) Program - Full-time employees earn 15–30 days of total PTO per year andpart-time employees may earn PTO on a prorated basis depending on years of service.Holidays - Eligible employees receive 11 paid holidays each year.Extensive Wellness - Employees are eligible for the Health Coaching program, which provideshelp from a trained health care professional 24 hours a day at no charge.
