controlled security protocols and a signed release, which must be printed directly from The CCI™ Solution.
• Sharing is triggered from a system-generated, multi-level participant release of information
• Information sharing is time bound and may be revoked by the participant at any time
• Aggregate reporting available for community wide outcomes, excluding individual identifiers
• Any information may be designated as private and cannot be shared with other providers, even with a signed release
• All data sharing protocol and technology is reviewed by legal specialists in intellectual property, privacy, computer
