      • IASP says:
      • A systems approach to workplace safety provides a means to determine specific needs of our workplace and our employees, as opposed to regulations that are designed in a broad and general fashion .
      • A systems approach to workplace safety will include comprehensive workplace safety audits and thereby meet and exceed government safety regulations .
      • The systems approach provides a site specific approach to workplace safety but not a comprehensive approach because it does not address the additional hazards or weaknesses created by the system itself . The law of unintended consequences in the system itself can be seen as a an example of looking beyond the commonly accepted bounds of a " Systems Approach " to workplace safety .
      • Therefore, the creation of a comprehensive safety culture within an organization requires that we step outside of the system, and evaluate the whole

    • Behavioral Safety is monitored
      • IASP says:
      • Behavior modification is defined as many different things, depending upon who is defining it . From the standpoint of the creation of a comprehensive safety culture within an organization, behavior modification means changing the manner in which the human element of our organization works .
      • This is accomplished largely through effective training, but also requires administrative controls .
      • Effective behavior modification within a comprehensive safety culture must apply not just to laborers and operators, but to all human links in the chain that is the " system ".

      • Consider… “8 principles of Safety”
      • Safety is an Ethical responsibility
      • At its core, ethics holds up a positive vision of what is right and what is good .
      • It defines what is " worth " pursuing as guidance for our decisions and actions .
      • Workplace injuries and deaths are too often seen in the abstract as statistics . But when it happens to someone we love, we suddenly see the reality of the horrible pain and suffering and its widespread effect .
      • It is our ethical responsibility to do what is necessary to protect employees from death, injury, and illness in the workplace .
      • This is the only foundation upon which a true safety culture can be established in any workplace .
    • 2. Safety is a Culture, Not a Program .
      • The combined commitment and participation of the entire organization is necessary to create and maintain an effective safety culture .
      • Every person in the organization, from the top management of the corporation to the newest employee, is responsible and accountable for preventing injuries .
    • 3. Management is Responsible
      • Management's responsibility is to lead the safety effort in a sustained and consistent way, establishing safety goals, demanding accountability for safety performance.
      • And providing the resources necessary for a safe workplace .
      • Managing safety is the responsibility of every supervisor, from the first line supervisor to the Chairman of the Board .
    • 4. Employees Must Be Trained to Work Safely
      • Awareness of safety does not come naturally; we all need to be trained to work safely .
      • Effective training programs both teach and motivate employees to be a productive part of the safety culture .
    • 5. Safety is a Condition of Employment
      • The employer must exhaust every reasonable means to lead, motivate, train, and provision employees to maintain a safe workplace .
      • But, in the event the employee refuses to take the actions required to work safely, the employer must utilize a system of progressive discipline to enforce safety requirements and ensure the cooperation of the employee, or the removal of the employee from the workplace in order to protect the employee and their coworkers
    • 6. All Injuries Are Preventable
      • Sometimes accidents occur without the apparent indication of fault or blame .
      • But there is always some chain of events that occurred leading up to the accident that, had we realized the eventual outcome, someone could have interceded .
      • The fundamental belief that injuries are, by their nature, preventable is a catalyst that encourages us to prevent injuries
    • 7. Safety Programs Must Be Site Specific, with Recurring Audits of the Workplace and Prompt Corrective Action
      • The purpose of the workplace audit is to discover and remedy the actual hazards of the site before they can injure workers .
      • Recurring hazard analyses, comprehensive inspections, and aggressive investigation of accidents or near misses, discover potential workplace hazards and identify weaknesses in safety plans, programs, policies, and procedures .
      • Safety regulations and generic safety programs are not sufficient means to discover hazards because they are not specific to the individual workplace .
      • A safety audit program is site specific . Whenever a safety deficiency is found, prompt action is required both to overcome the hazard and to reinforce the message that safety is a priority
    • 8. Safety is Good Business
      • Reducing workplace injuries and illnesses reduces the costs of workers' compensation, medical expenses, potential government fines, and the expenses of litigation .
      • Effective workplace safety is not an expense; it is an asset

    • HSE PLAN
      • I have seen many safety plans and programs that were merely a restatement of the CODE. These serve no useful purpose and do not meet the requirements of the CODE, nor of the COMPANY.
      • The Contractor must take the standard and make it specific to the SITE situation.
      • He will then write step-by-step procedures for the employees to use in order to safely perform the task.
    • CDM Regulations (UK/EU Law) 2007 for Contractors compliance….. specifies
      • Principal Contractors
      • The principal contractor is responsible for preparing and reviewing the Construction Phase Plan that identifies the risks to health and safety that arise from the work and the measures to address them. (This was formerly the duty of the planning supervisor.)
      • He/she will also co-ordinate the activities of all contractors to ensure that each complies with relevant health and safety legislation and the health and safety plan. To this end, the principal contractor may give information, instruction and site induction to other contractors and arrange for their training where appropriate.
      • The principal contractor must also establish a route for employees to raise health and safety concerns in order to fulfill the obligation for adequate consultation.
      • Finally, the principal contractor is responsible for liaising with other contractors and the CDM Co-ordinator on the content of the Health and Safety File .
      • (The duties of the principal contractor are covered under Regulations 22, 23 and 24).

      • Other Contractors
      • All contractors must plan, manage and monitor their own work so that risks to health and safety are minimised and take reasonable steps to ensure that work is carried out in accordance with the Construction Phase Plan.
      • The principal contractor must be kept informed of the appointment of any other contractors.
      • Contractors must also provide information for the health and safety file and tell the principal contractor about any injuries or incidents.
      • Other contractors must provide information for the Health and Safety Plan on risk assessments and control measures, and comply with site rules and emergency procedures.
      • (The duties of contractors are covered under Regulations 13 and 19).

      • Competence and Appointment of All Duty Holders
      • Although we have already mentioned the issue of competence in respect of selection of contractors by the client, there is now a need (under Regulation 4) to establish the competence of all persons involved in the construction project.
      • This covers the duty not to appoint a CDM Co-ordinator, designer, principal contractor or contractor unless reasonable steps have been taken to ensure that the person to be appointed is competent, and extends to not accepting such appointment themselves unless they are competent to do so.
      • No duty holder may arrange for a worker to carry out or manage any construction or design work unless the worker is competent to do so or is under the supervision of a competent person. Co-operation (Reg 5) and co-ordination (Reg 6) now apply to everyone involved.
      • All duty holders must now take account of general principles of prevention as set out in Schedule 1 to the Management of Health and Safety at Work Regulations 1999 in the performance of their duties and in the carrying out of construction work (Regulation 7).

      • SOLVAY HSE will work with the contractors HSE and Supervision (in partnership) to prepare a multi lingual safety induction briefing which all personnel must attend “and sign agreement to comply” (including visitors) before being issued with a site pass, or gain temporary access to the site.

      • Joint Emergency Response and evacuation procedures & drills
      • Muster points – and procedures to account for all personnel present
      • Only certified and approved and inspected construction equipment shall be allowed on-site (examples)

        • Scaffolding
        • Welding equipment
        • Approved electrical equipment
        • Compressors / generators
        • Rigging and slings (LOLER)
        • Hand Tools with test certificates
        • Etc
    • SITE SERVICES
      • HSE SIGNS, Exits and Mandatory compliance signs and site instructions will be erected and posted in Thai & English in prominent locations.
      • Site Offices (approved design with Ergonomic Std)
      • Amenities - Toilets / showers (For up to 1,000 workers)
      • Canteen & kitchens / rest areas
      • Warehousing – Storage, Loading & Unloading (approved design for safety)
      • Compressed Gas storage area (Approved design and procedures)
      • Chemical storage area (Approved design & procedures)
      • Waste materials and chemicals, collection, handling & safe disposal
      • Road layout – Parking – lighting (Designed for best safety)
      • Fencing (Approved) Consideration for CCTV
    • HAZARD IDENTIFICATION
      • SOLVAY HSE will perform a Construction Hazid with the Contractor / Subcontractors
      • Which will Identify critical activities by …….
      • Review of Client / Contractors design HAZOP and project execution planning
      • Review TASK RISK ASSESSMENT for major activities
      • Review COSHH assessments / and conduct routine Audits for the use and control of chemicals and Hazardous waste storage and disposal
      • Review “Competent persons” experience, qualification & training (and have the right to reject or require additional training)
      • Review Contractor Lift Plans
      • Participate in Contractor TRA’s & JSA’s
      • Review & Approve the PTW’s daily
      • Review & approve and routinely inspect LO/TO, Scafftag, Ladder tag, confined space entry etc.
      • Conduct, near miss, accident & Incident Investigation to determine the route cause, feedback to management & follow up on corrective actions
    • Risk Mitigation
      • Solvay HSE will ensure that all safety assessments follow the 3 golden rules .
      • First: Remove the Hazard identified (find another way to achieve the work, thereby removing the task completely)
      • Secondly: If the Hazard cannot be removed then look at implementing engineering controls (e.g. guarding etc.)
      • Finally: If Engineering controls still do not mitigate the hazard, then, and only then will administrative controls be considered such as personal PPE and specific task training to make the Task ALARP (e.g. working at height, confined space entry)

      • Review all contractors procedures and check that all contractors personnel are properly trained, understand and follow the SWP’s correctly
      • Observe the implementation of procedures and conduct routine inspections and audits on site
      • Monitor continuously with safety officers for any unsafe conditions, and potential hazards
      • Conduct training the trainer, when appropriate for the contractor
      • Monitor all key activities daily, attend tool box talks when necessary
      • Monitor noise and Environmental pollution issues, and ensure compliance
      • Monitor that Site Health facilities are maintained to standard
      • Monitor Health & Hygiene issues (kitchens and canteen / rest areas)
    • Specific HSE responsibility
      • HSE has the responsibility to STOP the work if any potential dangerous situation is seen, and to report the situation immediately (Not only HSE but everyone on the job also has this responsibility )
      • HSE has responsibility to conduct Health, Safety & Environmental protection training to International standards for Company and in partnership to assist and guide Contractors training program
      • HSE has specific duty to conduct an audit program and undertake routine daily inspections and report to the Project Manager
      • HSE has a duty to jointly develop the ER Plan and participate in the Joint Emergency Response Team and conduct joint drills

    • REMEMBER …. SAFETY IS EVERYONE’S BUSINESS ……. ZERO TOLERANCE IS A CULTURE WHICH STARTS AT THE TOP and involves everyone in the project: Client, Management Team, Designer, Purchasing, HR, Main Contractor, Subcontractors, Suppliers, Manufacturers / Vendors
