  • 3. Medication Reconciliation Goals: •Support implementation of MedRec for all owned and operated Residential Care facilities •Support development of clinical resources and tools •Provide and support opportunities for networking •Establish standardized metrics
  • 4. Medication Reconciliation Success! •MedRec webpage on BCPSQC site (resources, tools, contact information, etc) •All Health Authorities are currently reporting MedRec data for designated residential care facilities with good results •Now what?

  • 6. Medication Reconciliation We all have common needs/goals: •PharmaNet and Health Care Information System integration (Cerner /Meditech) •Collaborate with other related initiatives -48/6, CLeAR, Polypharmacy, Antimicrobial Stewardship •Accreditation requirements 2014 -2018
