  • 2. How satisfied are you with current insurance benefits as a package (includes office visits, urgent care, hospital care, Prescription cost) Please mark using this scale: 1 = Unsatisfactory, 3 = Average, 5 = Very Satisfied.
  • 3. If Willard R-2 Schools offered a similar benefit package which resulted in a savings of $500 redirected to your salary, would you be willing to change doctors and hospitals?
  • 4. With the cost of health care benefits rising, would you be willing to pay a portion of your medical insurance premium in order to keep the same level of coverage?
