    • How To Evaluate Ungraded Results?(CAP Surveys)Prepared By: BILAl Al-kadriJuNE, 2010
    • Proficiency Testing:
A form of External Quality Control
      Good Laboratory practices
Develops confidence in the accuracy & reliability of your results
CAP Surveys three times/year (some two/year)
Evaluate each analyte and specimen for negative/positive bias, trends or shifts
    • Documentation:
Always maintain documentation of CAP surveys for minimum 2 YEARS.
This includes Participant Summary, investigations. Reporting forms, evaluation reports, etc…
    • Documentation:
You can download Participant Summary for each surveys as necessary.
      www.cap.org
Refer to following slides for downloading
    • Action to take:
The College uses exception reason codes for the PT analyte that has not been graded,… (refer to next slides for various codes)
      • When exception reason is due to lack of participants Code 27, then the lab (you) should compare its (your) results against statistics supplied in the participant summary.
    • Exception Reason Codes:
    • Exception Reason Codes:
    • Evaluation Criteria:
      Analyte: Activated Partial Thromboplastin Time
      Target Value: Peer Group
      Eval Criteria: +/_ 15%
      • Manually or using excel sheet,
      • Calculate the Mean of the Means:
      52.9+51.6+49.2+…………..+53.7+46.7+46.9 = 55.8
      23
      Mean -15% is: 55.8 – (55.8 x 15/100)= 47.4
      Mean +15% is: 55.8 – (55.8 x 15/100)= 47.4
    • Evaluation Criteria:
      Analyte: D- dimer, Quant
      Target Value: Peer Group
      Eval Criteria: +/_ 3SD
      • Manually or using excel sheet,
      • Calculate the Mean of the Means:
      4797.6 + 2309.27 + 1591.01 + 2019.5+ 2028.86
      5
      Mean -3SD is: 2549.25 + (3x268.37) = 1744.14
      Mean +3SD is: 2549.25 + (3x268.37)= 3354.35
    • Evaluation Criteria:
      Analyte: Fibrinogen
      Target Value: Peer Group
      Eval Criteria: +/_ 20%
      • Manually or using excel sheet,
      • Calculate the Mean of the Means:
      269.605+261.488+…………..+271.382+267.904 = 296.177
      23
      Mean - 20% is: 296.177– (296.177 x 20/100)= 236.942
      Mean + 20% is: 296.177+ (296.177 x 20/100)= 355.412
    • Evaluation Criteria:
      Analyte: INR
      Target Value: Peer Group
      Eval Criteria: +/_ 20%
      • Manually or using excel sheet,
      • Calculate the Mean of the Means:
      2.49+3.37+3.17+…………..+2.64+2.71+2.86= 2.77
      23
      Mean - 20% is: 2.77– (2.77 x 20/100)= 2.22
      Mean + 20% is: 2.77+ (2.77 x 20/100)= 3.32
    • Evaluation Criteria:
      Analyte: ProthrombinTime
      Target Value: Peer Group
      Eval Criteria: +/_ 15%
      • Manually or using excel sheet,
      • Calculate the Mean of the Means:
      18.82+34.73+33.41+…………..+29.4+28.27+29.58=28.46
      26
      Mean -15% is: 28.46– (28.46 x 15/100)= 24.19
      Mean +15% is: 28.46 – (28.46 x 15/100)= 32.73
    • Conclusion:
      Code 27 – Lack of participant or referee consensus;
Document a self-evaluation following instructions as per the Participant Summary.
Every effort should be made to find the cause(s) of an unacceptable pt result.
