    • Expresses your desires for the handling of your financial and personal affairs when you die
    • Describes how you want your assets to be distributed to your heirs upon death

    • Begin answering the questions to prepare your MyCare Plan® documents
    • Note: It may be helpful to review the questions and gather any paperwork necessary before you start, although you will have the option of saving your work and returning to continue at any time.
    • Once completed, submit for final approval
    • Receive your MyCare Plan® documents via mail
    • Follow the enclosed directions to validate your final wishes
