  • FMEA – Analysis Cycle FMEA Cycle Step 1 – Detect Failure Mode Step 5 – Risk Priority Number Step 2 – Severity Number Step 3 – Probability Number Step 4 – Detection Number Step 6 – Action & Follow-up
  • Potential Failure Mode For each process input, determine the ways in which the input can go wrong - the failure modes
  • Potential Failure Effects For each failure mode, associate the inputs and determine the effects of the failures on the customer
  • Potential Failure Causes Identify potential causes of each failure mode
  • Current Controls – Prevent / Detect List the current controls for each cause or failure mode (Prevent / Detect)
  • Rating Scales - Severity
          • Severity = importance of effect on customer : 1= Not Severe, 10= Very Severe
  • Rating Scales - Occurance
            • Occurrence = frequency in which a given cause occurs and creates Failure Mode : 1= Not Likely, 10= Very Likely
  • Rating Scales - Detection
            • Detection - Prevent the causes or failure mode from occurring 1= Likely to Detect, 10= Not Likely at all to Detect
  • Risk Priority Number
            • Risk Priority Number (RPN) = Severity (effect) X Occurrence (causes) X Detection (controls)
  • Recommendation Determine recommended actions to reduce high RPN’s
