The first laparoscopic non anatomic resection of a focal nodular hyperplasia (FNH) was reported by Gagner and colleagues in 1992, and this was followed by the first report of laparoscopic anatomic liver resection in 1996. Since this time, improvements in laparoscopic devices have significantly extended the surgeon's ability to perform these procedures safely, as reflected in recent publications. Liver surgery comprises various operations of the liver for different disorders in India. The most common operation performed on the liver is a resection (removal of a portion of the liver). The most typical indication for liver resection is a malignant tumor. Tumors can be primary (developed in the liver) or metastatic (developed in another organ, then migrated to the liver). The majority of liver metastases come from the colon. The single tumor or more than one tumor confined to either left or right side of the liver can be successfully resented with 5-year survival as high as 60%. Liver resection patients are carefully evaluated by a multidisciplinary team to ensure the absence of the extra hepatic (outside the liver) tumor. Liver resections performed on patients with extra hepatic disease may relieve the symptoms caused by the tumor, but offer little improvement in survival. Laparoscopic Liver Cyst Drainage in India:
  • Cysts in the liver are frequently found in normal people. Liver cysts should only be treated if they are causing significant symptoms to the patient. The symptoms found with liver cysts include pain, bleeding into the cysts causing pain, and digestive complaints that are unexplained by other findings. If a liver cyst requires treatment, surgical treatment should include removal of the wall of the cyst. Removal of the fluid only from the cyst is not recommended since the cyst fills up rapidly after the procedure. Furthermore this procedure puts the patient at risk for infection of the cyst. We offer a laparoscopic approach to patients who require treatment of liver cysts. The procedure involves removing part of the wall of the cyst so that the liquid that is in the cyst can freely drain into the abdominal cavity. The body then removes the liquid from the abdominal cavity. Risk factors of laparoscopic liver surgery in India Include: 1. Gender: Men are more likely then women. 2. Liver Disease: This include hepatitis B and C infection. 3. Cirrhosis: it’s a disease caused by alcohol and hepatitis B and C. 4. Tobacco: The risk may be even greater for those who also consume alcohol. 5. Anabolic steroids: this is especially true after long-term use. 6. Birth control pills: The risk is small, and there are only studies showing a risk with pills no longer in use. Treatment for laparoscopic liver surgery in India: The three main treatments are Surgery (either by removing the tumor or transplantation), radiation, and chemotherapy. Combining tow or more methods is more effective. If the cancer cannot be cured (which can only be done through surgery), the goal may be to remove or destroy as much of the cancer as possible to prevent the tumor from growing, spreading, or coming back for as long as possible. Complete removal of most liver cancers is not possible, as the cancer is often large, found in many different parts of the liver, or has spread beyond the liver. Cirrhosis also can eliminate surgery as an option.
  • Open Surgery: When surgery is an option and a transplant is unnecessary, the standard procedure is open surgery. Surgeons make a large incision, which exposes the patient to cold, dry air. Surgeons remove the whole tumor, the lymph nodes, and anything else until the patient is left with normal physiological anatomy. Laparoscopic Liver surgery procedure in India: A handful of doctors are performing liver surgeries laparoscopically, or minimally invasively. Internally, it is the same operation, but it instead utilizes smaller tools with a small incision. That allows for a better operation, faster and easier recoveries, and patients can start subsequent cancer treatment sooner, if necessary. No patient was excluded and patients were selected for laparoscopic surgery as having a lesion which was technically possible to resect or treat laparoscopically whilst leaving an adequate resection margin where applicable. In the case of patients with hepatic tumors, a standard multidisciplinary team approach in regards to preoperative staging, treatment, and selection was adopted. Conclusion: Laparoscopic liver surgery has transformed the approach of many surgical procedures, reducing associated pain, hospital length of stay, and achieving comparable results with open operations. Skilled surgical teams, with experience in hepatic and laparoscopic liver surgery in India, have demonstrated that the laparoscopic approach for liver resection is safe and feasible for a selected group of patients with benign or malignant liver tumors. Additional studies are needed to assess the long-term outcomes following laparoscopic hepatic resection.
