Medical Tourism, the idea of visiting to a new destination for health treatment and visiting some favorite tourist places at the same time sounds very relaxing and refreshing. But traveling to new places and finding a suitable hotel to stay and a right physician to get treated is very difficult and challenging issue. The main reason of the foreign patients to visit Indian hospitals is the low budget treatment for the Laparoscopic ectopic pregnancy in India. Ectopic pregnancy is one of the abnormal outcomes of pregnancy in 2% of pregnant woman and is defined as implantation of a fertilized egg outside the endometrial cavity. It remains a major cause of maternal morbidity and mortality when left untreated and accounts for as much as 9% of maternal death in this country. Quantitative measurements of the beta subunit of human chorionic gonadotropin (ß-hCG) and transvaginal ultrasonography have improved the accuracy of diagnosis and allow earlier detection of ectopic pregnancies. Ectopic means ‘out of place’. An ectopic pregnancy is when a pregnancy starts outside your uterus instead of inside. The embryo often develops in either of your Fallopian tubes. This is called a tubal pregnancy. If not tubal, the pregnancy could be in your ovary or some other part of your pelvis. An ectopic pregnancy is dangerous because of the risk of very serious internal bleeding. It needs to be dealt with quickly. According to surgeons in India, an ectopic pregnancy occurs when the baby starts to develop outside the womb (uterus). The most common site for an ectopic pregnancy is within one of the tubes through which the egg passes from the ovary to the uterus (fallopian tube). However, in rare cases, ectopic pregnancies can occur in the ovary, stomach area, or cervix. Laparoscopic surgery of ectopic pregnancy is done to stop blood loss in India. An ectopic pregnancy is usually caused by a condition that blocks or slows the movement of a fertilized egg through the
  • 2. fallopian tube to the uterus. This may be caused by a physical blockage in the tube. Most cases are a result of scaring caused by: Past ectopic pregnancy, 2.Past infection in the fallopian tubes, 3.Surgery of the fallopian tubes. Up to 50% of women who have ectopic pregnancies have had swelling (inflammation) of the fallopian tubes (salpingitis) or pelvic inflammatory disease (PID). Some ectopic pregnancies can be due to: 1.Birth defects of the fallopian tubes, 2.Complications of a ruptured appendix, 3.Endometriosis, 4.Scarring caused by previous pelvic surgery. Symptoms of laparoscopic ectopic pregnancy: 1. Abnormal vaginal bleeding 2. Amenorrhea 3. Breast tenderness 4. Low back pain 5. Mild cramping on one side of the pelvis 6. Nausea 7. Pain in the lower abdominal or pelvic area If the area of the abnormal pregnancy ruptures and bleeds, symptoms may get worse. They may include: 1. Feeling faint or actually fainting 2. Pain that is felt in the shoulder area 3. Severe, sharp, and sudden pain in the lower abdomen. Internal bleeding due to a rupture may lead to shock. Shock is the first symptom of almost 20% of ectopic pregnancies. The Benefits of laparoscopic ectopic pregnancy: This operation is often lifesaving. An ectopic pregnancy will usually bleed until it is removed. Keyhole surgery has several advantage over an open operation in India: 1. The laparoscopy can confirm the diagnosis and the exact position of the ectopic pregnancy. 2. There is less bleeding from the operation with less chance of needing a blood transfusion.
  • 3. 3. Less pain. 4. Recovery is usually quicker with a shorter hospital stay. 5. The chance of having a normal pregnancy later is higher, about seven in 10 (70%) rather than five in 10 (50%) after open surgery. 6. The chance of another ectopic pregnancy is lower, about three in 20 (15%) rather than five in 20 (25%) after open surgery. Alternatives of laparoscopic ectopic pregnancy: We can sometimes treat an ectopic pregnancy by giving a drug called methotrexate. We usually give this drug as an anti-cancer treatment. When we give it to treat an ectopic pregnancy it stops the embryo from growing any bigger. The embryo dies and the body slowly absorbs the remains. This can avoid the need for surgery. If you need surgery you could have an open operation, called a laparotomy. Due to the benefits of laparoscopy over open operations, it is usually better to avoid this. If for some reason a laparoscopic ectopic pregnancy operation is not possible, Surgeon in India will recommend an open operation. One reason for this would be if you were very ill due to internal bleeding from the ectopic pregnancy. Prevention of laparoscopic ectopic pregnancy: Most forms of ectopic pregnancy that occur outside the fallopian tubes are probably not preventable. However, a tubal pregnancy (the most common type of laparoscopic ectopic pregnancy) may be prevented in some cases by avoiding conditions that might scar the fallopian tubes in India.
