A liquid nitrogen spill occurred at a liquid nitrogen gas fuelling station, resulting in the suffocation of 4 workers/facility commissioners.
The spill occurred during the offloading of liquid nitrogen into the storage tank for the commissioning process. The liquid outlet valve was opened accidentally resulting in a large release of liquid nitrogen. Multiple workers/commissioners ran into the bunded/diked tank farm area consecutively, trying to close the valve. This resulted in 4 fatalities due to nitrogen asphyxiation.

The spill occurred during the offloading of liquid nitrogen into the storage tank for the commissioning process. The liquid outlet valve was opened accidentally resulting in a large release of liquid nitrogen. Multiple workers/commissioners ran into the bunded/diked tank farm area consecutively, trying to close the valve. This resulted in 4 fatalities due to nitrogen asphyxiation. The attempts by personnel to rescue their colleagues without any form of Respiratory Protective Equipment would have been futile. As little as one breath of pure N2 is sufficient to cause unconsciousness and then, very soon after, death. CONSEQUENCES People 4 fatalities Environment none Equipment/material none Product/process none Media/authorities local authorities, no extra information available concerning media CAUSES Immediate causes asphyxiation in a nitrogen cloud Probable root causes under investigation CURRENT SITUATION - WARNINGS  Cryogenic Nitrogen storage is available at many of our sites. In some cases these storage capacities belong to a contractor (Air Liquide, Air Products, Praxair, etc.). The accident shows that a sudden accidental release of liquid nitrogen can result in very severe consequences. La fuite a eu lieu au moment du déchargement d’azote liquide vers le réservoir de stockage pour le processus de commissioning. La vanne de sortie pour l’azote liquide a été ouverte accidentellement et a causé une émission importante d’azote liquide. De nombreux travailleurs sont rentrés dans l’encuvement du parc de stockage, pour essayer de fermer cette vanne. Ceci a abouti à 4 décès par suffocation à l’azote. Les essais pour sauver des collègues sans utilisation d’équipement de protection respiratoire étaient vains. Une aspiration d’azote pur est suffisante pour que la personne perde connaissance et décède un peu plus tard. Dans certains cas, ces stockages appartiennent à un fournisseur (Air Liquide, Air Products, Praxair, etc.). Cet accident montre qu’une fuite accidentelle importante peut avoir des conséquences très importantes.
