Meridian Terms and Conditions Meridian Terms and Conditions Document Transcript

  • Meridian Programme Terms and Conditions valid from 01 September 2008 0. General The Meridian Frequent Flyer Programme (hereinafter Meridian/ Programme) is a loyalty programme operated by AeroSvit – Ukrainian Airlines (hereinafter AeroSvit). All benefits that AeroSvit and Meridian partner airlines offers in the Meridian programme are conditioned upon your compliance with these Membership Terms and Conditions, all terms and conditions of AeroSvit's and Meridian partner airlines contract of carriage, AeroSvit's and Meridian partner airlines fare rules, and all other applicable AeroSvit and Meridian partner airlines rules and regulations. These rules are subject to change at any time. Unless otherwise stated, the current rules in effect at the time of your travel, request for a benefit, or other transaction will govern that transaction. AeroSvit reserves the right to use partner companies in specific countries as joint operators of the Programme. Meridian offers an opportunity to earn and redeem miles with various participating service providers (airlines, hotels, car rental companies, car sales companies, banks, restaurants, mobile operators etc.). Please check back frequently for Programme updates or rule changes. 1. Programme Membership Meridian Programme membership is free of charge. Meridian Programme is open to any person aged from 2 and older in any country that has not prohibited participation in frequent flyer Programmes. Corporations, associations, legal entities and other groups cannot be enrolled as members. 1.1. Commencement and termination of Membership Membership commences upon enrollment by means of filling and signing an Application Form, when a personal mileage account is opened with Meridian. His/her signature confirms that the member is familiar with and agrees to the Meridian Terms and Conditions. Members must provide full name to participate in the Meridian Programme. Full name means the given name, middle initial or middle name, if any, and family or surname. The full name must be the same as the name on the member's passport or other valid travel document. Application Form for registration of new member aged from 2 till 16 years should be signed by both parents of member or one of parents (thus it is meant that the parent who has signed the Application Form has preliminary received the consent to its signing from other parent). Upon necessity the Airline reserves the right to request copy of documents which confirm related communications with parents of Programme’s member till 16 years. Completed and sent electronic Application Forms available at the AeroSvit/ Meridian partner airline website or telephone applications made through the Meridian Service Center are also considered to be valid applications, on the terms of specified above including children - members of the Meridian Programme aged from 2 till 16 years. Only one person may be enrolled per Meridian account. Members may not maintain more than one Meridian account. Membership accounts are nontransferable. In the event more than one account number is assigned to the same individual, the duplicate account will be cancelled, and only the applicable mileage credit, excluding any enrollment bonus, can be transferred to the remaining account. As evidence of the opened account, the customer receives a Meridian card number which entitles the member to earn miles. The Meridian card remains the property of AeroSvit and must be surrendered to AeroSvit upon request. AeroSvit/ Meridian partner airline reserve the right to reject applications without giving reasons. Each Meridian member may terminate his/her membership by writing an application for membership’s termination to the Meridian Service Center (Meridian Frequent Flyer Programme, #7a, 9/2, V. Vasylkivska Str., 01004, Kyiv, Ukraine). Such a written application should contain his/her frequent flyer number, name, last name, date of birth , date of membership termination, signature and a copy of a member’s passport. Membership with a temporary card will be automatically terminated in case there is no movement on the account over the period of 48 months. 1.2. Personal Identification Number Each member receives a PIN for personal identification, e.g. when ordering awards or checking the online account balance. To avoid misuse, the member must ensure that the PIN is not disclosed to unauthorized third parties. AeroSvit Ukrainian Airlines 2008
  • Meridian Programme Terms and Conditions valid from 01 September 2008 Member is responsible for the selection and use of his/her PIN and for maintaining the confidentiality of his/her PIN. Member may not divulge his/her PIN to anyone. If you he/she does, he/she is solely responsible for any actions that person takes with his/her PIN, including, without limitation any transactions in his/her account, any violations of the Meridian Programme rules. AeroSvit/ Meridian partner airline shall have no liability for losses resulting from unauthorized access to, or use of, member’s PIN. 2. Miles 2.1. General Miles form the basic unit of all calculations for all frequent flyer accounts to reward those Meridian members for their loyalty and trust to AeroSvit and Meridian partner airline. No bonus miles or miles under other Programmes may be converted into Meridian miles, unless Meridian has reached an agreement with the other issuing Programme operator concerned. Miles and the mileage accounts are non-transferable, and no refund is possible. 2.2. Account balance Members may check their current account balance at Representative of AeroSvit, Meridian partner airline’s office or Meridian Service Center and via their personal online service by entering their Meridian card number and PIN on www.aerosvit.com. 2.3. Mileage accrual Miles may be earned from the date of commencement of membership. The number of miles credited depends on the Programme conditions valid at the time of utilization of the service. Meridian also offers mileage accrual with specified partner companies. Since Meridian has no direct control over these companies, neither AeroSvit nor the joint operator concerned may be held responsible for availability of services provided by partner companies (e.g. flights, hotel stays, rental cars) and for proper performance of any contracts. 2.3.1. Flights Miles are credited to the member's account for each actually flown segment of a fully paid scheduled flight by AeroSvit or Meridian partner airline. The amount of miles credited for direct flights depends on the distance between point of departure and destination, as indicated on the ticket, on separately announced minimum miles or on fixed rates announced in the Meridian communication media. The mileage is multiplied by the booking class factor according to the flight class paid and indicated on the ticket. Otherwise, the published minimum miles or fixed rates are credited. AeroSvit and Meridian partner airline reserves the right to change the applicable multiplier and booking classes for the mileage credit for certain destinations. The Programme provides the possibility to credit bonus miles to a member for all regular AeroSvit and Meridian partner airline flights flown within 6 months before the actual date of registration in the Meridian Programme. In such a case a member should present to the Meridian Service Center the following flight documentation: original/ copy of passenger’s receipt and original/ copy boarding passes for each segment flown. Meridian reserves the right at any time to deduct miles for a flight/service which has not been realized, or where mileage accrual evaluation does not apply, or where it is a discrepancy between what it was added and what should have been added to the Meridian member’s account. Miles for a particular flight/service may only be added once. In case where it is possible to include one flight/service into more marketing Programmes only one Programme must be chosen. 2.3.2. Non-airline partners Each member may earn a fixed number of miles when choosing the services of non-airline Meridian partners. Miles are credited to the member's account for every paid stay or night at a Meridian partner hotel, provided the booked hotel rate qualifies for mileage accrual at the time of conclusion of the contract for AeroSvit Ukrainian Airlines 2008
  • Meridian Programme Terms and Conditions valid from 01 September 2008 accommodation. The same applies to every paid rental of a vehicle with a Meridian car rental partner, provided the booked rental car rate qualifies for mileage accrual at the time of conclusion of the contract of hire. The number of miles credited is announced separately. Two or more consecutive overnight stays are treated as "one stay". The same applies if a member checks in at the same hotel within one day of checking out. "One car rental" is defined as the rental of one vehicle for at least one day. Miles are also credited to the member's account when purchasing mobile operator or car sale company service. The number of miles credited is announced separately. 2.3.3. Meridian Credit Card with Mileage Accrual Function Holders using their Meridian Credit Card in payment earn a specified number of miles which are credited to their Meridian account. Further details are contained in the General Terms and Conditions of the Meridian Credit Card. 2.3.4. Other Ways to Earn Miles Other ways to earn miles and the relevant conditions are announced separately in the Meridian communication media. 2.3.5. Exclusions from Mileage Credit Miles can not be earned on irregular flights (charters) and tickets endorsed with NO FFP/NO MILEAGE ACCRUAL. No miles are credited for certain booking or service classes, Industry Discount Tariffs (ID, IP, AD etc.), award services, free flights or services for which the member receives benefits under another bonus Programme. Certain other services may also be excluded from mileage accrual following a prior announcement in the Meridian communication media. 2.3.6. Crediting Procedure Automatic crediting of miles to a member’s account is only possible after the actual flown regular AeroSvit or Meridian partner airline flights provided that member quotes Meridian card number qualifying for miles accrual while making a reservation, check-in or purchasing a ticket. Mileage is not credited for unused, reimbursed, lapsed or unlawfully obtained documents (tickets, flight coupons). The same applies to other unused or refunded services that qualify for mileage accrual. For any reason it is recommended to a member to keep his/her flight documentation (passenger’s receipt and boarding pass) after each actual flight flown. 2.3.7. Retroactive crediting procedure Any claims for miles not automatically registered can be credited to the member's account within 6 months after commencement of a flight/service concerned. For this purpose, Meridian Service Center requires the complete documents: original/copy of passenger’s receipt plus original/ copy boarding passes for each segment flown, or original/ copy invoice/ agreement for using Meridian partner’s services etc. The subsequent data checks and confirmation is to be proceeded within 14 days after the flight documentation received by Meridian Service Center, in case when original boarding passes are missing it takes 30 days. The subsequent data for using Meridian partner’s services checks and confirmation is to be proceeded within 30 days after the documentation received by Meridian Service Center. There is no guarantee from Airline’s or Meridian partner airline’s side for crediting of miles for a flight or service claimed when both passenger’s receipt and boarding passes are missing or in case receipts on non-airline partners’ service are not provided. AeroSvit and Meridian partner airline secures the right to keep all documentation received from the Meridian member what is based on the reporting purpose for retroactive crediting miles. 2.4 Mileage redemption 2.4.1 General Members may redeem their miles and order awards as soon as their mileage account has reached the balance required for the award concerned, subject to the availability of the awards pursuant to item 2.4.6.
