    1. FAULTY HEALTH RECORDS AT VETERAN HEALTH CENTERS By Yannis Lemmens Head of I.T. department
    2. What has been affected by this?
        • These faulty health records have caused software malfunction at the Veterans Affairs health center around the United States.
        •   The result of this are incorrect drug doses, treatment and/or  other medical errors.
        •   These faulty health records could be life threatening to the patient as they got wrong medicine doses and treatment, but no fatalities has occurred.
    3. Symptoms of these glitches
        • Medical data popped up under another patients name on the monitor
        •   The computers also gave stop order for a treatment, this lead to cases of unnecessary doses of drugs
            •   One day nine reported cases were made public where patients in Veteran health centers got incorrect doses of heparin drips that were given 11 hours longer than normal
        •   Other cases involved other medical mixtures and were given 15 hours longer than planned. 
        •   The doctors were positive that they didn’t harm their patient’s.
        • The glitches began when the Veteran affairs health center upgraded their software in August and all the glitches lasted until December
    4. Information Technology at Veteran health centers
        • Nearly 1/3 of all veteran health centers reported to see some malfunctions on their monitor
        • VA’s acting secretary for information technology said that they decreased the number of glitches by checking on alternate systems
        • By upgrading their databases the glitches began
        • The hospitals only began reporting the reports in October, about 2 months after when the malfunction started.
    5. The Future…
        • Safety checks are taking place regularly in order to improve the patients safety
        • Notify personnel if glitches are occurring
        • Create back ups of databases every 2 weeks
