  5. 5. • Createdin 1989 to address workforce shortages inunderserved rural and urban communities•Awards up to $100,000 for education loanrepayment in return for three years of service in anunderserved area
  6. 6. • Physicians (MD or DO) providing primary care(FP, IM, Peds, OB) and Psychiatrists•Dentists, Hygienists, NPs, PAs, CNMs•Working full time in a Health ProfessionalShortage Area, a federally qualified healthcenter, tribal health center or rural site for threeyears •Full time: 32 clinic hours/week, 45 weeks/year (21 hours/week for OB)

Physicians are eligible for either:•Up to $50,000 if practicing in a HPSA, or•Up to an additional $50,000, if practicing in a ruralHPSA, for a total potential award of $100,000, or•Up to $50,000 if practicing in a rural area but not in aHPSA. •Rural: population less than 20,000 and at least 15 miles from anywhere that has a population of 20,000+
  8. 8. • Three annual payments• Two party check to you and your lender• All awards are state and federal income tax exempt and require a three year service obligation
  9. 9. Federal designation based on data provided by theWisconsin Department of Health ServicesDesignation based on: • Population : Provider Ratio • Poverty level • Distance to nearest provider • Specific need factors (high % elderly, alcohol use, etc.)The majority are rural
